  • 2021 LEXUS RX
    - Hilton Head Island, SC

    The tire (Bridgestone Ecopia 422 Plus 235/55R 20 102V M+S) failed within seconds while driving on the highway at 70 MPH. Luckily I did not lose control and was able to steer the car from the middle lane to the birm thanks to the semi driver on my right who allowed us to get off the road quickly. There was almost no rubber remaining on the rim and the tire pressure sensor was gone. The tow truck driver and NTB tire technician said the tire appeared to have failed due to delamination of the steel belt. Subsequent searches on the internet seem to support belt failure as the cause of the rapid loss of the tire. Walmart has refunded the cost of the failed tire. Bridestone is being less than helpful relative to the remaining three tires, which have less than 2,000 miles on them.

  • 2024 LEXUS RX
    - Fairburn, GA

    The contact owns a 2024 Lexus RX. The contact stated that while idle at a red stoplight, the entire vehicle began to violently jerk back and forth. No warning lights illuminated. The vehicle was taken to a local dealer, where it was diagnosed with a software malfunction, and the failure was replicated. The vehicle was repaired. Medical attention was sought for severe depression, anxiety, PTSD, memory loss, concussion, head, neck, and back injuries. No police report was filed. In addition, the contact stated that after the injuries, she was no longer able to work or drive. The manufacturer was made aware of the failure. The failure mileage was approximately 22,790.

    Brakes suddenly failed in the middle of driving causing a major safety hazard. There was no warning. Went to the dealership and was diagnosed “Brake - Fluid Accumulator: INSPECT AND FOUND CODE C1391, ABNORMAL LEAKS IN ACCUMULATOR, RECOMMEND TO REPLACE BRAKE BOOSTER, MASTER CYLINDER AND PUMP. The brake fluid accumulator is a part of the Anti-Lock Brake System. The accumulator stores brake fluid under pressure from the ABS Pump and can instantly provide fluid under pressure to the brakes.” It appears this issue is not an isolated incident and others with the same or similar year/model have had this issue. It is extremely dangerous that the brakes suddenly failed to function without warning. The brake pads and brake fluids were adequate and the vehicle was up-to-date on regular inspection and maintenance.

    I am submitting a formal complaint regarding Lexus’ refusal to correct a known safety-related defect under Customer Support Program (CSP) 21LE01. My vehicle meets all criteria outlined in the program, yet Lexus denied coverage. Details: 1.Program Coverage: CSP 21LE01 covers Brake Booster and Brake Booster Pump assemblies for internal malfunctions verified by Diagnostic Trouble Codes C1391, C1252, C1256, or C1253. Coverage extends 10 years or 150,000 miles, whichever comes first. 2.Vehicle Qualification: •Mileage: under 150,000 miles •Model year/production range: covered under CSP 21LE01 •Symptoms: brake-system warning lights, reduced braking performance, and internal malfunctions •DTCs: C1391, C1252, C1256, C1253 confirmed 3.Lack of Notification: Lexus indicates that owners were notified of this program starting May 2021. I never received any notice, despite my contact information being current with the DMV. 4.Denial of Repair: Despite the vehicle qualifying under all CSP 21LE01 criteria, Lexus informed me I am “not eligible” for the repair. This refusal directly contradicts the program’s published requirements. Safety Concern: This defect affects the brake system, a critical safety component. Denying the repair for a qualifying vehicle exposes the driver and others on the road to potential brake failure and increased risk of accident. If the brake booster/pump fails while driving, braking can become hard, delayed, or fail completely, greatly increasing the risk of accidents, collisions, injury, or death. Request: I am requesting that the National Highway Traffic Safety Administration investigate Lexus’ denial of a safety-related repair and compel the manufacturer to honor the repair coverage under CSP 21LE01 for my vehicle.

  • 2020 LEXUS NX

    I am submitting this complaint to report a significant safety defect in my Lexus vehicle. Without any prior warning or history of mechanical problems, the vehicle experienced a sudden and severe loss of engine power while I was merging onto a busy highway. The vehicle rapidly decelerated despite continued acceleration input, forcing me to navigate to the shoulder while surrounding traffic was traveling at high speed. This unexpected loss of power created an immediate and substantial risk of a collision and posed a serious danger to myself and other motorists. Following this incident, I took the vehicle to a local auto shop, which identified diagnostic code P030002, indicating a misfire in Cylinder 2. I then promptly brought the vehicle to an authorized Lexus dealership. Their diagnostic confirmed the same code and attributed the failure to a “Cylinder 2 direct fuel injector leaking.” Lexus has already acknowledged this defect in TSB L-SB-0007-21, demonstrating that the manufacturer is aware of this recurring issue. My vehicle is five years old with fewer than 100,000 miles, yet this defect resulted in a dangerous loss of engine power under highway conditions. The dealership quoted over $3,000 to replace a single injector, and the remaining injectors may fail at any time, further compounding the safety risk. Despite its knowledge of this defect, Lexus has not issued a recall, implemented preventive measures, or offered assistance, citing that the vehicle is slightly outside the 70,000-mile warranty. I have therefore been left with a vehicle that is unsafe for continued highway operation. Given the severity of the incident, the known recurrence of this defect, and the manufacturer’s lack of corrective action, I respectfully request that NHTSA investigate this matter, as it presents a substantial and ongoing hazard to consumers and the driving public.

  • 2018 LEXUS ES

    The contact owns a 2018 Lexus ES 350. The contact stated that while his daughter was driving at an undisclosed speed, the vehicle jerked abnormally. The driver was able to drive to the residence. Upon test-driving the vehicle, the contact noticed that the vehicle was hesitating and jerking. The contact stated that the vehicle made an abnormal clicking sound while driving the following day, and the vehicle later became undrivable. There was no warning light illuminated. The vehicle was towed to an unknown dealer, where it was diagnosed with transmission failure. The vehicle was not repaired due to the cost. The manufacturer was notified of the failure, but no assistance was provided. The failure mileage was approximately 200.

  • 2004 LEXUS RX
    - Fresno, CA

    Dashboard crack in 2024, that started on passenger side near air vent, expanded recently causing the "passenger air bag off" alert to light up and stay on continually. If I were to have a passenger at the time of an accident, the air bag might not deploy, which would be a safety issue. This could possibly lead to a liability issue, so I have not had any passengers but that isn't sustainable, so I contacted the dealer. I was told that the previous recall for cracked dashboards ended in 2017 and that it was cosmetic only, not safety related, but that the part was available for $964.50, not including labor, tax, etc. I feel that a safety related recall should be issued, the previously known dash cracking problem didn't end when their recall did, the fact that my vehicle is primarily garaged preserved the interior integrity, I don't believe I should be penalized for my dash not cracking 8 years earlier.
