2024 MAZDA CX-90 PLUG-IN HYBRID- Decatur, GA
The contact owns a 2024 Mazda CX-90. The contact stated that the vehicle failed to start up after several attempts due to an issue with the hybrid battery. The contact stated that the message "Electrical Leakage in the Hybrid System” was displayed. Due to the failure, the vehicle was towed to a dealer where the service technician acknowledged that the failure and confirmed that the issue with the hybrid battery was a known issue. The vehicle was not diagnosed or repaired and remained with the dealer. The manufacturer was not notified of the failure. The failure mileage was approximately 24,000.

2024 MAZDA CX-90 PLUG-IN HYBRID- Los Angeles, CA
On two separate occasions, my young children were locked inside the vehicle, and on one of the two occasions my elderly mother was also locked inside. In all incidents, the doors and windows remained closed and would not unlock, even when attempting to use the key fob. Specifically, after parking, I exited the driver’s seat and closed my door. When I went to open the rear doors to get my children out of their car seats, the rear doors were locked and could not be unlocked by the key fob. The same occurred when my elderly mother was in the vehicle. Each time, the car was in hot conditions, and we were fortunately able — after some struggle — to unlock the doors and remove them from the vehicle. This presents a serious safety risk, particularly in warm weather and with young or elderly passengers who cannot exit the vehicle themselves. I am currently pursuing a lemon law claim for this vehicle, but I also wanted to formally report this defect because of the danger it poses.

2019 MAZDA CX-5- Batavia, IL
For the 3rd or 4th time since I have own this car I experienced a loss of acceleration when attempting to make quick acceleration while attempting to merge with traffic. Acceleration is fine around town where quick acceleration is not appropriate. But the first attempt to merge with flowing traffic the car will slowly accelerates to about 30 mph and no more regardless as to position of accelerator pedal. On all occasion this bad behavior has this first drive of the day - going to work in the a.m. There is often a warning message indicating the safety radar is not working due to limited visibility which is a very common warning in the a.m.

2024 MAZDA CX-30- College Station, TX
I have received a recall notice back in June 5th. I tried to reach my dealer (Douglas Mazda) couple of times but they are not fixing it. As it has been announced that, it is not available. The vehicle is under warranty. PLease suggest me what steps I should take? As it is not not safe drive in any highway.

2024 MAZDA CX-90 PLUG-IN HYBRID- Eagle Point , OR
My wife called me while I was at work today to let me know that our soon to be [XXX] old toddler locked himself in the car with her keys inside while the car was running with the air conditioning on. She repeatedly tried to use the My Mazda app to unlock the car but it kept giving an error message. She called 911 and luckily he finally unlocked it on his own just prior to the police department's arrival. Upon further research this appears to be a known issue that has been brought up to Mazda from other owners and it has been dismissed by Mazda as a safety feature designed to prevent theft. Apparently this is by design, the app will not let you unloco the car if the vehicle is running with the keys in it. Personally, the theft of the vehicle is nowhere near as important as the health and safety of my child. We were extremely fortunate that the air conditioning was on as the temperatures in our town are hitting triple digits right now. What if it had not been on? This could have had a much worse outcome. Additionally, how in the world does this deter theft? So what Mazda is basically saying is that somebody would get my phone, access my password/fingerprint protected app, unlock my car from my phone, then get in and drive off. The likely hood of this happening is way lower than a curious toddler or rambunctious dog accidentally getting locked inside the running vehicle. Additionally, if my wife locked her keys in the car, and isn't home to get the spare, or im at work and can't bring her the spare right away, she's [XXX]. 2019 MAZDA CX-5- Stamford, CT
I am the owner of a 2019 Mazda CX-5 (non-turbo) which experienced a cylinder head leak requiring full replacement. The repair was done by Riley Mazda of Stamford on July 28, 2023, at a personal cost of $4,074.34. Mazda has acknowledged this defect in turbocharged CX-5 models from the same year and issued a reimbursement program. However, Mazda has declined reimbursement for non-turbo models, even though the failure is the same, and the repair used identical part numbers and labor hours as listed in Mazda’s own TSB for the turbo reimbursement. This indicates a broader issue affecting both engine variants. Mazda’s refusal to extend reimbursement to non-turbo owners is unfair and leaves affected customers with no support for a known manufacturing flaw. I urge NHTSA to investigate this recurring engine defect in non-turbo 2019–2020 Mazda CX-5 models and encourage Mazda to offer equal reimbursement.

2018 MAZDA CX-5- San Diego, CA
Infotainment system acts randomly, changing to max volume during driving, startling driver. Also changes stations, skips audio programs and distracting driver. Rear view camera has intermittently failed to show video feed when driving in reverse.

2025 MAZDA CX-50 HYBRID- Harker Heights, TX
Due to a hybrid battery malfunction, full body sensor mlafunction and TPMS functionality error, vehicle left inoperable. Vehicle could not be placed in neutral. Vehicle could not be jump started.

2024 MAZDA MAZDA3- Chico, CA
On July 1, 2025, I was involved in a severe rear-end collision while driving my 2024 Mazda3 manual hatchback. The impact was forceful enough to shatter the rear windshield, leave a deep dent in the tailgate, and bend the trailer hitch mount under the vehicle by several inches. Despite this, none of the airbags deployed, and there were no airbag warning lights before or after the crash. After the incident, I checked my vehicle’s VIN on NHTSA’s recall lookup tool and discovered that Recall 7525E had been issued on June 6, 2025, for a critical airbag sensing system defect (SAS module), which can disable airbag deployment after battery depletion or improper power conditions. According to Mazda and NHTSA documents, this defect can completely disable airbag function, increasing the risk of injury in a crash. As of July 11, 2025, I have not received any recall notice from Mazda by mail, email, phone, or dealer contact—despite being the registered owner. Mazda’s documentation shows that: Dealers were notified as early as June 3, 2025 Owner letters are not scheduled until July 28, 2025 This means that I was driving a defective vehicle without knowledge of a serious safety issue at the time of the crash. I believe that: Mazda failed in its legal obligation to notify me in a timely and adequate manner. The non-deployment of airbags in my crash is consistent with the failure described in Recall 7525E. Other affected Mazda owners may remain unaware and at significant risk. I respectfully request that NHTSA: Investigate whether Recall 7525E was the cause of airbag failure in this incident. Determine whether Mazda’s recall notification practices met regulatory requirements. Ensure immediate outreach to affected owners to prevent further injury or risk. I am also seeking legal counsel due to the serious safety implications and potential liability arising from this defect. I will be retaining all documentation, vehicle records, photos, and post-

2024 MAZDA CX-50- Rutland, VT
The contact owns a 2024 Mazda CX-50. The contact stated that there was uneven brake wear on the inner brake pad. The contact stated that the inner side of the brake pads were wearing before the outer sides. There were no warning lights illuminated. The vehicle was taken to the local dealer, where it was diagnosed and determined that the front brake pads and rotors were defective and needed to be replaced. The vehicle was not repaired. The manufacturer was not contacted. The failure mileage was approximately 2,500.
