  • 2022 MAZDA CX-5
    - Melissa, TX

    Humming sound coming from driver’s side headlight as soon as unlock bottom pressed in key fob. This sound comes from a faulty cooling fan inside the headlight and is a known defect. Humming sound progressed to very slow pitch indicating that fan function is diminishing over time. Mazda will replace the headlight but required me to pay $200 toward it. This should be a recall with replacement at no cost to the consumer as they are aware of the defect.

  • 2021 MAZDA CX-5
    - La Crosse, WI

    The contact owns a 2021 Mazda CX-5. The contact stated that while driving 70 MPH on the interstate and changing lanes, the blind spot warning light was inoperable. The contact stated that the blind spot indicator then became operable. An independent mechanic was notified of the failure and stated that the repair was not covered. The local dealer was contacted, where a prior unknown diagnostic test was completed; however, the blind spot failure had not been addressed. The vehicle was not repaired. The manufacturer was not contacted. The failure mileage was approximately 95,000.

  • 2016 MAZDA CX-5

    Repeated Failure and Rebooting of Mazda Connect Infotainment System I am the owner of a 2016 Mazda CX-5. The factory Mazda Connect infotainment system has developed a recurring defect in which the system repeatedly reboots to the Mazda logo while driving. Prior to complete failure, the navigation system became intermittent and Bluetooth connectivity frequently disconnected, requiring repeated attempts to reconnect devices. The system now continuously cycles through the startup screen, making navigation, hands-free phone functions, and system settings inaccessible. These repeated reboots occur while the vehicle is in motion and are distracting to the driver. The flashing and rebooting screen repeatedly draws the driver's attention away from the road and interferes with the use of navigation and hands-free communication features that are intended to promote safe driving. After researching this issue, I found numerous reports from other Mazda owners experiencing the same symptoms involving the Mazda Connect system, navigation SD card, and Connectivity Master Unit (CMU). The frequency and similarity of these reports suggest that this is a widespread and well-known defect rather than an isolated incident. Because this defect affects driver attention and the safe operation of the vehicle's communication and navigation functions, I respectfully request that Mazda and the National Highway Traffic Safety Administration investigate this matter and require Mazda to issue a recall or establish a repair program for affected vehicles.   • 2026 MAZDA CX-50

    At approximately 2,900 miles, during the first oil change/service, the factory-installed spin-on oil filter could not be removed normally. The oil filter required approximately one hour of removal effort and would not spin off in a normal manner. When the filter was finally removed, the oil filter threaded adapter/union came out of the engine-side oil filter housing while still seized inside the oil filter. The adapter threads showed visible damage, including flattened, galled, and torn thread areas. The oil filter was seized tightly enough to the adapter that the oil filter had to be cut away from the adapter to separate the parts. The adapter was removed from the filter without adding further damage. The damaged adapter threads were cleaned/chased only as a temporary repair because the vehicle was needed for work and parts were not immediately available over the weekend. A new correct replacement oil filter was test-fitted to the adapter. It threaded fully onto the adapter by hand but had slight binding in the previously damaged thread area. The filter was installed, the engine was started, and the assembly was checked for leaks. No leak was observed during the temporary service check. However, the adapter remains damaged and should be replaced. This appears to be a manufacturing, assembly, or original filter installation issue, because the vehicle had only approximately 2,900 miles and the factory-installed oil filter was seized to the oil filter threaded union. The filter should not be seized to the adapter at this mileage, and the threaded adapter should not be pulled out of the engine-side housing during normal oil filter removal. The concern is that a damaged or improperly threaded oil filter adapter could cause oil filter sealing failure, oil leakage, loss of engine oil pressure, engine damage, or an oil leak onto the roadway. The damaged threaded oil filter adapter/union should be documented as a factory/dealer assembly defect.

  • 2020 MAZDA CX-5

    Cylinder Head has failed at 53,000 miles. Mazda has a technical service bulletin acknowledging that this is a common occurrence with this part. They have an extended warranty for the turbo engine but not for the standard engine. Looking through online forums, there are hundreds of not thousands of these issues for this model year and engine. Mazda has clearly acknowledged there is an issue and should repair these manufacturing defects that are not normal wear and tear items.

  • 2024 MAZDA CX-5
    - Davenport, FL

    On 06/18/2026, the vehicle displayed multiple safety warnings, including ABS malfunction, dynamic stability control/traction control malfunction, Smart Brake Support and Forward Obstruction Warning disabled, possible forward sensing camera malfunction, and airbag/seat belt pre-tensioner malfunction. These warnings involve braking, stability control, collision avoidance, airbag, and seat belt safety systems. This was not the first occurrence. On 04/24/2025, the vehicle displayed similar warnings, including critical brake system/Electronic Brake Force Distribution malfunction, ABS malfunction, Smart Brake Support and Forward Obstruction Warning disabled, possible forward sensing camera malfunction, and keyless entry malfunction. The vehicle was towed at that time. The vehicle was inspected by an authorized Mazda dealer in June 2026. The dealer replaced the battery under warranty, but I was not provided diagnostic trouble codes, a root-cause explanation, or written confirmation that the braking, ABS, airbag/seat belt, stability control, Smart Brake Support, Forward Obstruction Warning, and forward sensing/camera systems were fully diagnosed and verified safe. The dealer inspection report also listed the mileage as “No Mileage.” The vehicle is available for inspection upon request. I do not feel safe driving it because the repeated warnings involve braking, collision avoidance, stability control, airbags, and seat belt systems. I am concerned that an electrical, braking, or safety-control defect may disable safety systems while driving.

  • 2021 MAZDA CX-9

    Check engine light comes on and error P0303 comes on. Had an independent mechanic do the following and after each one, error P0303 still came on 1. Swap the ignition coils from 2 to 3 2. Swap spark plugs from 2 to 3 3. Swapped injectors 4. Compression test 5. Leak down test 6. Changed the spark plug, coil and injectors 7. Looked for vacuum leaks. Mechanic believes that it is related to the cracked cylinder head in the recall, MC-10232269-0001.pdf. My vehicle is just outside of the recall range and has 87k miles on it. Other research online shows other people with the same vehicle and similar milealge are also having this problem.

  • 2017 MAZDA MAZDA6

    The center mounted Mazda Connect infotainment system in my 2017 Mazda 6 Touring sedan is defective. The screen randomly & intermittently changes display modes while the vehicle is on (driving or parked). It began about 2024 & getting worse. One second it will be on navigation then it arbitrarily switchs to phone settings or set a random destination in the navigation screen or change radio channels. the routine is entirely random and is never the same results. There are no dash warning lights, no EWL, no indications of any kind before, during or after. Resetting the infotainment system does not resolve the issue. road conditions or speed do not change the issue. outside temperature seems to affect the unit more because the hotter the ambient temp, the worse the issue seems to be. likewise, high humidity & high temperature causes more issues. The vehicle is garaged in South Florida. removing the SD card from the center console (the navigation maps from Mazda) does not affect the issue. replacing the Mazda navigation SD card does not make any change in the frequency, duration or intensity of the issue. moving the jog dial on the center console will allow interfacing the infotainment system, however, not touching the jog dial still has an issue. The issue is becoming a hazard while driving because the turn-by-turn navigation randomly routes to random destinations and when cancelled, it randomly picks another point of interest and starts all over again. The radio cannot stay on a single frequency, it scans through presets. The issues are distracting and the vehicle is out of factory warranty. The dealership cannot reproduce the issue after 3 separate visits for regular maintenance. The reverse camera (also using the screen) works normal most of the time. I had a single issue where the camera would not turn on while in reverse. disconnecting the main 12v battery reset the camera and it has been working since that isolated issue. (Case No. [XXX] (W.D.
