When you turn, the steering wheel doesn't retract or turn back on it's own. You have to give it tug or jerk, which makes it dangerous.

2019 MAZDA CX-9- Aldie, VA
My 2019 Mazda CX-9 (VIN [XXX]) suffered catastrophic cylinder head failure on the SKYACTIV-G 2.5T engine at approximately 124,000 miles. The failure is consistent with Mazda TSB 01-013/21 (and successors 01-007/22, 01-010/24), in which Mazda acknowledged that cylinder heads on this engine can crack at the stud bolt hole and exhaust manifold flange due to manifold deformation and residual production stress. The first warning came in mid-May 2026, when the engine temperature warning illuminated during normal driving. An independent mechanic replaced the thermostat. The warning returned within a week, and the thermostat was replaced a second time with a higher-quality part. Four to five days later, the warning returned alongside more severe symptoms: the vehicle began jerking while in motion, a strong coolant-burning smell developed, and smoke was visible from the exhaust pipe upon shutdown. The vehicle became undriveable, had to be left at the failure location, and was towed. Safety risk: The jerking compromised vehicle control in traffic. Upon shutdown, smoke and burning coolant odor created fire and fume-exposure risks. Had this failure occurred at highway speeds or in heavy traffic, the sudden loss of engine power and erratic behavior could have caused a serious collision. Family members were in the vehicle during several escalating events. Confirmation: The vehicle was towed to an authorized Mazda dealership, where Mazda-certified technicians confirmed in writing that it is exhibiting the exact defect described in TSB 01-013/21 and that complete engine replacement is required. The diagnosis is formally documented and has been communicated to Mazda's warranty team. Mazda Customer Experience has been engaged since mid-May 2026 and acknowledged the issue. 2018 MAZDA CX-5- NEWBURYPORT, MA
Cylinder head suddenly cracked, causing heavy oil leakage onto hot exhaust manifold. Car started to smoke heavily. Vehicle turned off before potential engine fire.

2018 MAZDA CX-5- DEL MAR, CA
I am submitting this complaint regarding a cylinder head failure on my Mazda CX-5. The vehicle has approximately 53,000 miles and has been properly maintained throughout its service life. The vehicle developed a crack in the cylinder head, which was subsequently confirmed by an authorized Mazda dealership. Mazda has previously issued a Technical Service Bulletin (TSB) addressing cylinder head cracking on certain vehicles; however, my vehicle's VIN is not included in the covered range despite experiencing the same failure described in the TSB. The dealership initially provided a repair estimate of $8,789 plus tax to replace the cylinder head. Following further discussions, the dealership reduced the repair cost to $3,903 plus tax. They recognize that this crack is not part of normal wear and tear but due to the VIN number not being included in the recall are unable to provide full coverage. A cylinder head crack is a major engine failure that should not occur on a well-maintained vehicle with only 53,000 miles. I am concerned that vehicles outside the VIN range identified in Mazda's TSB may be experiencing the same defect and that the scope of the issue may be broader than currently recognized. The vehicle was purchased used from the same Mazda dealership that diagnosed the failure and is currently performing the repair evaluation. I respectfully request that NHTSA review this issue to determine whether additional Mazda CX-5 vehicles may be affected by the same cylinder head defect despite not being included in the existing TSB.

2024 MAZDA CX-90- Dallas, GA
The contact owns a 2024 Mazda CX-90. The contact stated that while driving 70 MPH, the dynamic cruise control feature failed to function as intended. The contact stated that the vehicle slowed down and then independently accelerated. The contact depressed the brake pedal to stop the vehicle. The message “Steering Assist Unavailable” was displayed. The vehicle was taken to the dealer; however, the failure was not duplicated. In addition, the contact stated that the vehicle was taken to several dealers, however the failure was not duplicated. The contact informed the dealers that the vehicle needed to be driven on the expressway for the failure to occur. The contact stated that the failure did not occur while driving the vehicle at slow speeds on the local streets. The contact stated that the failure was persistent while driving on the expressway. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 11,000.

2011 MAZDA MAZDA6- Omaha, NE
The contact owns a 2011 Mazda Mazda6. The contact stated that while driving at an undisclosed speed and coming to a stop, the steering wheel was not operating properly. Several days later while the contact's wife was driving at an undisclosed speed, there was play in the steering wheel. Additionally, there was an abnormal sound coming from the passenger’s side wheel. The contact stated that the wheel was turning abnormally. A mechanic arrived at the residence to diagnose the vehicle and determined that the sub frame was rusted. While touching the sub frame, pieces of the sub frame fractured. The dealer was notified of the failure. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 92,000.

2026 MAZDA CX-5- Seattle, WA
About a week ago, a "telematics error" message displayed, a single red light on the SOS button turned on, and it started beeping loudly. This has repeated multiple times. In this state the vehicle cannot automatically summon emergency help following a crash, and I cannot manually trigger an SOS call. More recently, June 17th, a new symptom has occurred. Vehicle's touchscreen display would not turn on. This display is required to access heating/cooling features including defrost/defog, and is used to show the backup camera. The dealer has informed us that these issues were all caused by a manufacturer-initiated over-the-air software update. We are currently scheduled to have a new software update installed to replace it on June 22nd.

2021 MAZDA CX-30- Oakland Park, FL
The vehicle experienced a premature internal A/C failure, resulting in a total loss of climate control refrigerant. Because the A/C system failed, the vehicle's windshield defroster completely stopped removing humidity and moisture from the cabin air. This caused the front windshield to instantly and severely fog over while operating the vehicle completely obstructing forward driver visibility. The manufacturer has issued Technical Service Bulletin (TSB) MC-10203760-0001 for factory evaporator defect on 2021 models, acknowledging low refrigerant charge due to component leakage. However, the manufacturer's current TSB VIN cutoff is too narrow. My vehicle is a 2021 model built at the same facility experiencing the exact same component failure past the arbitrary cutoff, proving the dangerous defect extends into later production runs. The current parameters must be expanded to protect public safety and a recall needs to be initiated to fix the issue.

2016 MAZDA CX-5- Rolling Meadows, IL
The contact owns a 2016 Mazda CX-5. The contact stated that while driving 70 MPH with the cruise control activated, the cruise control cancellation switch failed to operate as intended after several attempts or while depressing the brake pedal. No warning light was illuminated. In addition, the contact was able to deactivate the cruise control using the main cruise control power switch. The vehicle was taken to a local dealer to be diagnosed; however, the cause of the failure was not yet determined. The vehicle was not repaired. The manufacturer was made aware of the failure by the local dealer. The failure mileage was approximately 91,000.

2019 MAZDA MAZDA6- Ft Thomas, KY
Coolant Control Valve was not working per recall !! They put in a dummy plug and it did not fix which should have . Based on your research in using the dummy plug . I asked the dealership could be the dummy plug installation and asked if there waa a system update .. I hope you can help me get car back on road for the coolant control valve !! Florence ky Mazda said. They could not do nothing else !! Please help resolve ty
