Safety issue, random, with 360 View Monitor cameras malfunctioning. This camera issue has happened randomly at least 7 times since I purchased the car in May 2023. The most recent occurrences were 10/26, 09/27, 09/23, 08/03/2023. What happens: when starting the car, 360 View Monitor should show images of surroundings. Instead, no image from the cameras shows; it’s black on the screen. Colored lines show the direction the car is backing up or moving forward, but the rest of the screen is black. When it occurs, it will remain in this faulty condition for the entire drive. The button on the console (left of gearshift) that turns on the image cameras when driving will show the same image. It will remain in this disabled mode until the car is turned off for a long time. I’ve tried turning it off and letting it sit until the computer sounds like it has shut off (2-3 min, perhaps), and restarting the vehicle — this does not cure the problem. The car seems to need to be turned off for a longer time. If I turn the car off and return in, say, 2 or more hours, the issue is cured (temporarily). Attached are still images from a video. This is random in occurrence and seems to be completely unpredictable when it will will happen. Safety issue.

Problem noticed shortly after purchasing the car in May 2023, and is a constant issue. Always: when driving, and when I press the the button to the left of the gearshift to activate the 360 View Monitor, the default image shows a useless, mostly black, image. (Scrolling to the other two, non-default images, they are useful images.). Instead, it gives the appearance of a black gap in the space around the car. I am attaching several still images from a video. Safety concern, because the image is not correct, and this is a listed safety feature.

When using adaptive cruise control and passing a large trailer or truck in the left lane (the trailer or truck is in the right lane), the adaptive cruise control system incorrectly detects the trailer or truck to be in the same lane as the CX-90 and in some situations can slam on the brakes causing a potentially dangerous situation.

Noticed the smell of burning oil in the cab on 30 mile drive. No warning lamps or other symptoms; vehicle continued to operate normally. After parking and inspecting the engine for the burning smell, noticed oil dripping from underneath engine bay and a pool of motor oil was forming on the ground underneath the vehicle. The next day, vehicle was brought to local Mazda dealer for inspection. They diagnosed the cause and stated the engine needs a cylinder head replacement. Oil leakage was so prolific, if this were to occur during a long distance drive, the engine would have lost all of the oil and stranded motorist - safety concern. Cylinder head failure is a known issue (Mazda TSB [XXX] ) with this engine. This known safety issue should trigger a recall. At minimum, the repair should covered by manufacturer for life of vehicle. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The adaptive cruise control sometimes misreads the driving situation, especially on curves, and applies strong braking when there is no vehicle ahead in your lane. Especially when the vehicle in the adjacent lane is a truck. This could easily lead to being rear ended by a close following vehicle that is not expecting you to abruptly slow down. I had to turn it off on a trip on mountain interstates. I will address this at my first oil change at the dealer. I have had adaptive cruise on several other vehicles and never experienced this problem.

We were at a stop light and we were rear ended by a Lexus SUV. We're assuming she was going 45mph + but we're not too sure yet. I couldn't figure out why myself, the drive of the car that was hit, had such a horrendous neck ache after being hit. I turned around & saw that my headrest had been completely ripped out of the seat itself, providing no support for my head and/or neck on impact. I can't find any information of if this is on purpose, as a safety feature, or if the headrest failed.

I have taken the Mazda CX9 since I bought it new to service at Mazda every 6 months or 5,000miles as suggested. Two years a new engine was required due to overheating and coolant problem which the service man noticed 5 months prior during service appointment but didn’t suggest doing anything. Now two years later new engine they installed overheated also. The Mazda has never been in a wreck of any sort. The Mazda has 86,000 miles on it only. How can a car have 2 engines fail in 7 years when it has been serviced there at the agency punctually as required?

The REAR subframe has rusted out pretty much completely, but the total failure of the right rear spring mount has allowed the spring to punch through the frame, being stopped only by the top plate which has now turned sideways and is lodged between the spring and the floor pan. I understand this is a common problem with this and a few other model years, and has not been addressed by Mazda. I am hoping to help change this.

The "infotainment" system, which includes media, navigation, hands-free calling, and the backup camera function, frequently malfunctions. The system will either freeze, or it will execute button presses on its own. The result is that it will change audio selection, make hands free calls via the connected phone, etc. on its own with no driver interaction, which is distracting while driving and could and has caused hazardous driving conditions. While I am listing the most recent occurrence in this form, this has been occurring on nearly every drive for the last 2 years. This typically happens in stop-and-go traffic. The vehicle has a manual transmission and the movement in the center console caused by shifting may be causing loose electrical connections to be disturbed and contribute to the behavior.
