Sunroof glass failure that occurred on my 2024 Mazda CX-5. While driving to work on May 9th at 11:16 pm going 67 miles per hour on route XXX in [XXX] , I suddenly heard a very loud popping/explosive sound from above the vehicle. Upon safely pulling over, I discovered that the sunroof glass had shattered unexpectedly. There was no known object strike that I or my two passengers observed, no prior cracks or chips visible before the incident, and no warning signs of damage beforehand. The glass appeared to have exploded outward/ upward and appeared curved upward. Most of the glass we believe hit the vehicle behind us as it was gone by the time we were able to safely pull over. It was approximately 58 degrees outside with no inclement weather. The vehicle currently has approximately 53,000 miles and has been properly maintained. The incident was extremely startling and created a significant safety concern while driving. Luckily, the interior sun visor was closed when this incident occurred, preventing the glass from entering the vehicle and injuring myself or my two passengers. Because the vehicle is relatively new and the damage appeared sudden and unusual, I would sincerely appreciate Mazda reviewing this matter for possible warranty or goodwill assistance. I have always valued Mazda’s quality and reputation, and I am hopeful the company can help support resolution of this unexpected issue. I have attached photographs of the damage and would be happy to provide any additional information needed. I would also appreciate the dealership documenting whether any definitive impact point or external strike evidence was identified during inspection. Thank you very much for your time and consideration. I look forward to working together toward a resolution. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

When adjusting the temperature in the car with the dial, it does not respond correctly. It either jumps up or down by multiple degrees. This causes to take your eyes off the road to try to adjust the temperature. No warning lights came on and pushing lighter or harder on the dial does NOT fix it. Only driver side is currently having this issue.

We purchased this vehicle used from Mazda of Orange in July 2025 with 52,399 miles on it. After 6 months of ownership and approximately 3,000 miles there was a burning smell. We took it to our local mechanic and the following repairs were required: Replacement of cracked belts, Replacement of cracked axle boots, Replacement of an excessively loose front axle, Replacement of valve cover gasket. Cost was $3,000 and the dealer refused to reimburse us. Now the car is leaking oil and needs a new head gasket and cylinder head surface machined. Quote was approximately $3,500. We have requested Mazda perform this repair. Will you please look into these vehicles? These are significant defects on a car with such low miles. We also believe that some of these defects were know to the dealer but are unable to prove it. We believe our vehicle is part of a batch that has signficant mechanical defects. Current mileage of the vehicle is 56,865.

My 2018 CX5 just hit 50,000 miles recently and I smelled something burning and saw smoke coming from the hood while I was stopped for a school bus. I immediately stopped driving due to fire concerns. The vehicle has a cracked cylinder which is a known issue for this vehicle. Mazda has no plans to correct this issue other than issuing a technical bulletin. I feel that Mazda should repair this at their cost because it is a defect and the vehicle only has 50,000 miles. Instead I have to pay nearly $5,000. This is hard to believe! Transcript from dealer: The technician reported an oil leak from the cylinder head, specifically indicating a crack in the number four lifter valley that is causing fluid to leak down the engine block. This issue is recognized as a known problem, and the technician will check if it is covered under warranty. Repairing the cylinder head will require significant labor, including dropping the engine, and will take some time to complete. Services being performed: - Oil change. - Inspection for oil leak issue. Recommendations: - Replacement of the cylinder head due to the identified oil leak. Multi Point Inspection $4870.22

The contact owns a 2011 Mazda Mazda6. The contact stated that while driving at 25 MPH, the contact attempted to swerve out the way of another vehicle as she suddenly lost control of the vehicle. Due to the failure, the contact force to steer the vehicle to the left as the contact attempted to maintain control of the vehicle. The contact managed to pull over into a parking lot and called her husband for assistance. Despite the failure, the contact managed to drive the vehicle home despite the vehicle being extremely difficult to maneuver. The contact's husband managed to drive the vehicle to a local mechanic, who determined that the front subframe had failed due to excessive corrosion of the subframe. The mechanic then informed the contact of NHTSA Campaign Number: 19V323000 (Structure, Suspension); however, the contact was informed that her vehicle was not included in the recall. The manufacturer was notified of the failure where they confirmed that the vehicle was not included in the recall. The vehicle remained in the mechanic's possession and had yet to be repaired. Hybrid battery recall was replaced and failed a month later for a second time burning other systems and causing a compete failure in the car. It failed in a deserted portion of [XXX] while traveling from [XXX] to [XXX] for important business. This left me stranded in the middle of the [XXX] desert late at night in a dangerous area. It failed on the highway loosing complete power with multiple failures reported on dashboard. I was left stranded in the middle of nowhere with no nearby services or immediate assistance and was forced to remain in the vehicle overnight in unsafe conditions. The vehicle required towing, and I was later informed the issue was again the hybrid battery the same component that had already been replaced under recall less than one month prior. This repeated failure creates a serious safety concern because the vehicle can unexpectedly lose power and leave occupants stranded without warning, even in remote or unsafe locations. In addition it lost power on the highway and that posses a significant safety concern. The vehicle is unreliable given its age and repeated failures, and I am requesting investigation into a potential safety defect involving the hybrid battery system and sudden loss of propulsion/power and damages to other parts. I had 8 different warnings on my dashboard before the car completely died and I couldn’t even open or close windows or lock and unlock doors. Hybrid battery failed causing electrical issues. - Fix in October 2024. During this time Service repair for the recall related to the power steering. Car became jerky and would lose the ability to hold turns. Took to mazda and told there was a gear housing need to be replaced. Was quoted at 4000 for the repair due to car having 49000. Mazda customer service stepped in and dropped the price in half. Eventually cover the majority of the cost as we reported this as a safety issue and it only camp up after the recall was performed for the power steering and the suspension was fixed. In the service report, Technician stated that the steering has fuse to the sway bar. I am now very concerned the sway bar is unsafe. Mazda did not comment on this when I ask. It appears as the fix one thing more continue to break. with The issues the lane assist continues to have issues seems to struggle with alignment issues. Car was with Mazda for 7 weeks during the 1st year of owning it. I am also concerned with some of the finding as I look at them with details pertaining to original purchase and miles being reported on our documents. Living in Kentucky the Lemon law does help with providing any sort of real action plans on CPO. Break have been squeaky causing uneven feedback. This issue has worsened over time. Car was purchased CPO - in April of 2025.

Component/System: Steering System (Electronic Power Steering / Steering Rack) What happened? I am experiencing a recurring and escalating failure of the steering system on my Mazda CX-90 PHEV. Currently, the steering wheel momentarily "locks up" or experiences a sudden, significant increase in resistance (stickiness) during left turns, especially when a large steering angle is applied. This occurs intermittently and without any warning lamps or error messages on the dashboard. Safety Risk: This failure poses a severe safety risk as it hinders my ability to complete turns smoothly and predictably. The sudden resistance requires unexpected physical effort to overcome, which could lead to a loss of vehicle control or a collision, especially in tight maneuvers or emergency situations. History & Inspection: Previous Symptoms: Approximately one year ago, the vehicle exhibited a "grinding/squeaking" noise when the steering wheel was turned to full lock. Prior Repair: A dealership inspected the vehicle and performed a recall repair (related to Recall 24V-022) by applying grease to the steering components. This temporarily resolved the noise. Recurrence: The noise has recently returned, and the new issue of "momentary locking" has now emerged. Inspection Status: The vehicle has been previously inspected for the related recall, and the current malfunctioning system is available for inspection upon request. Additional Context: I am reporting this because I believe the initial recall fix (grease application) was insufficient, and the vehicle is now demonstrating the "sticky steering" symptoms currently under NHTSA investigation (RQ26002).
