2020 MAZDA MAZDA3- Herndon, VA
I was on the Queensboro Bridge headed from Queens, NY into Manhattan on the afternoon of August 29. Weather was partly cloudy and dry conditions. Traffic was bumper to bumper and moving anywhere from 0mph to maybe 5mph. I was in the middle of the bridge driving approximately 5mph and my 2020 Mazda 3 beeped (collision warning) and applied brakes. I was almost rear ended and my two passengers were jolted forward. I maintained a safe following distance the entire time. I believe a van on my left triggered my car’s actions as it passed. I was in no danger of getting into an accident although I was quite shaken up since this is the first time my car applied its brakes. I have an appointment to bring my car in and would like the automatic braking system disabled. Previously, the car from time to time has alerted of a collision always under 40mph and a previous inspection of my vehicle (after I reported collision warning activation) revealed everything was working properly.

2011 MAZDA MAZDA6- Winchester, KY
Vehicle was brought in for recall 2818I in January 2020. Still noticed a rough ride afterwards and believed issue to be with the rear suspension. Local independent auto repair shop inspected suspension and said all was well. Brought in for regular check up at Mazda dealer in September 2021. Mechanic took video to show the amount of rust on the rear subframe. Condition is beyond repair without replacement and is hazardous to drive. This should have been addressed with the front subframe recall. Definitely needs to be addressed by Mazda now.

2016 MAZDA MAZDA3- Centerport, NY
The infotainment system fails to work - it is where I can access information re: maintenance schedule, or any info that is associated with a lit engine light. In addition the charging system for my mobile phone has ceased working. This includes both the usb ports and the lighter port which can be adapted with a charger that works in that receptable. These are all safety issues as far as I am concerned. Not having information related to the vehicle engine light is a big issue, but so is the fact that I can't charge my phone on long distance drives. If I needed assistance, I would be unable to contact anyone without a working cell phone. Because the display also controls the ability to pair a phone with blue tooth, there is concern for other drivers of the Mazda 3 (mine is a 2016) will be using the phone regardless of the bluetooth malfunction. I have seen a safety recall on other Mazda's with relation to the infotainment screen, but I do not see one here. I wonder why? To repair this crucial component would cost many hundreds of dollars -- something I am not in a position to do. Mazda claims this infotainment center is not under warranty, but with so much important information and safety features embedded into it, I think it should be corrected by recall. 2017 MAZDA MAZDA3- Wichita, KS
The contact owns a 2017 Mazda Mazda3. The contact stated while depressing the brake pedal, the instrument panel display was intermittently inoperable. The contact stated no warning light was illuminated. The contact took the vehicle to the local dealer, where it was diagnosed with needing the instrument panel to be replaced. The vehicle was not repaired but was pending repairs. The manufacturer had been informed of the failure. The failure mileage was 40,000.

2019 MAZDA CX-5- San Francisco, CA
1) It happened last week on 8/30, when I drove back home after work at night. After I parked my vehicle at the driveway and I had noticed there's smoke going into the cabin through the vent. The smell was very bad, it smells like burning rubber/plastic type. Then I went underneath the car and checked, the oil dripping out from the plastic panels where the oil pan is. Next morning, I jacked the car up and have a deeper inspection. I have notice the oil actually leaking from the Timing belt/chain cover area. I immediately called local dealership @ Putnam Mazda and drove it in for service on 8/31. When I arrived to the service department, my whole front end was full of smoke coming out from the hood. After a hour of inspection, service advisor told me that my car is having a blown cylinder #3 and oil leaks from cracked cylinder head. They will have to order a new cylinder head from Japan for my replacement, which will take more than 2 weeks or even longer due to part shortage. This is ridiculous!!! My car only have 21,500 miles on it, and it's a 2019 model year. The advisor told me they had quite a few were having the same issue. I guess he doesn't want to give me the exact numbers. But this is a safety concerns for other Mazda customers as well. I guess it has to do with the cylinder deactivation they put into the engine which cause this problem. The recalls that they had last year was not a fix or cure. 2) This is a safety concern and putting my life on risk, the oil leaks might cause fire or create accident on the road while operating. The smoke that I suck into my system may cause permanent damage to my lung and my health concern. 3) Problem had been confirmed with dealership. Mazda should declare a recall. You can't just wait for someone have an accident or cause their life to correct the issue. 4) The vehicle is at the servicer dept. and been inspected. 5) This is critical, I don't have any engine light or any indication. Even my scanner detected no code.

2021 MAZDA MAZDA3- Marina, CA
The contact owns a 2021 Mazda Mazda3. The contact stated that while driving at various speeds the passenger front side tire was flat which caused the contact to veer to the shoulder. The contact towed the vehicle to the dealer where the tire was inspected as the technician informed the contact that the tire was on backorder and no replacement tire was available. The vehicle then was towed back to the contact resident however was informed by the towing company that the vehicle abrupted in flames on the flatbed. The fire department was contacted and extinguished the flames. No police report was made. no injuries were sustained. The contact is awaiting approval from the insurance adjustor on adjustments needed to be made for the vehicle. The manufacturer was not notified of the failure. The failure mileage was approximate 2,509.

2014 MAZDA MAZDA3- Appleton, WI
Had a gas smell in our garage for months, thought our riding mower or snowblower was the cause but realized it was this car. Took to our mechanic and they said it was the fuel leak issue from NHTSA campaign 16V684000 however this car was manufactured in Dec 2013 and the official recall campaign began with cars manufactured the following month aka Jan 2014. We have been unable to get assistance from the dealership or reach anyone at Mazda due to the huge hold times.

2010 MAZDA MAZDA5- THONOTOSASSA, FL
The glue and seal around back windshield glass is separating. Glass is loose and rattles. This appears to be an ongoing issue with Mazda. The defects can cause window to separate while driving resulting in flying debris hitting another vehicle.

Multiple paint chips on Ref Soul Mazda. Dealer did not want to address issue. I would have to pay out pocket. The hood and sides of car.

2016 MAZDA CX-5- Lakewood, CO
We are having a consistent issue with our son kicking the harness release button located by his feet in the car seat. On numerous occasions we have gone to retrieve him from the seat and the harness is completely loose by kicking and releasing the trigger. This is very VERY unsafe. We have been in contact with the manufacturer via email and they have not responded to our request to remedy the situation. They advised that he may have outgrown the car seat, however he is 8 lbs under the weight limit. Additionally, the warning label on the seat says that they have not outgrown the seat until their head reaches the top of the headrest. He has come nowhere close to reaching this size. Britax offered to exchange the seat. Car seat has been replaced with another brand.
