  • 2010 MAZDA CX-9

    Front suspension broke while my husband was driving back from a fire as he is a volunteer firefighter. He states car did not have any lights on dash to report there was a problem. The next thing he knew he heard a loud squealing noise and lost control of the vehicle taking him off road in a small ditch. As the axle broke it went upward and hit both belts and shredded them. He was not able to move or drive the car at all. I am the owner of this vehicle had I been driving I would have totaled and crashed,also I do have A [XXX] son I drive to school most days. Thank goodness my husband is a heck of a driver. I have contacted the Mazda manufacturer and they are stating that the vehicle was already fixed that they will not do a safety remedy. As I found out there was a 8515G recall for the front suspension interior corroding. I just purchased this vehicle on 2/24/2026 it broke on 3/19/2026. The vehicle states to have been fixed in 2017. I have the inspection papers from the previous dealer from whom I bought it from in 2021 and the one from the dealer that I purchased it and that inspection was in 2025. There has been no inspections after I purchased or after the catastrophic failure happened I am in process of this and my insurance is the next I'm contacting. There was no police report as there was only damage done to the vehicle,luckily no one else was involved and my husband was ok.   • 2026 MAZDA CX-90

    The contact owns a 2026 Mazda CX-90. The contact stated that while driving at undisclosed speeds, the steering wheel was pulling to the left, causing the vehicle to merge into the left lane unintendedly. The contact firmly held the steering wheel to prevent the failure. Additionally, the vehicle jerked to the right lane while the lane keep steering assist function was activated because the steering wheel was pulling to the left. No warning light was illuminated. The vehicle was taken to the dealer, but no cause of failure was found. The failure persisted, and the vehicle was taken back to the dealer, and an alignment of 0.1 degrees was performed; however, the failure recurred. The vehicle was taken back to the dealer for the third time, and the contact was informed that no repair was available and was advised to contact the manufacturer for assistance. The vehicle was not repaired. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 100.

  • 2025 MAZDA CX-90
    - Chicago , IL

    Seatbelt got stuck between seat and seat frame. Seatbelt was stuck in the locked position and was neither able to be retracted or pulled out rendering the seatbelt unusable. This is a huge safety concern as we were stranded at night with small children and unable to use the seatbelt. A recall should be made to this vehicle (Mazda cx90). Further Mazda is claiming that seatbelts are not part of the warranty (it’s a <1 year old car) and they are charging $1000 to fix this. This is a huge safety issue to not have a functioning seatbelt and a design flaw that should be fixed. It is too easy under normal use for this to occur.

  • 2024 MAZDA CX-5
    - Sunnyvale, CA

    Incident Description: On multiple occasions in January and February 2026, while operating a 2024 Mazda CX-5 in reverse on low-friction surfaces (snow/ice) with a slight incline, the vehicle suffered a total systemic failure. System Failures: While reversing at low speeds, the engine spontaneously shut off without warning. This resulted in an immediate loss of power braking and power steering. Simultaneously, the electronic gear selector (shifter) became non-functional, preventing the vehicle from being placed into 'Park' to stop the uncontrolled rearward slide. Safety Risk: The vehicle became a "runaway" object on several occasions. In one instance, the vehicle narrowly avoided crushing a pedestrian against another parked car, stopping only when the tires reached a different surface texture. In a separate incident, the vehicle slid uncontrolled down a residential hill, striking snowbanks. There is a grave concern regarding whether the Supplemental Restraint System (airbags) remains functional during these engine-stall events, as the dealer could not confirm occupant protection status during a total power loss. Inspection & Reproduction: The vehicle was presented to an authorized Mazda dealer on March 3, 2026. The service department attempted to replicate the issue by reversing over soapy water on a gravel surface. However, due to the high friction of the gravel, they were unable to induce the specific tire-slip/low-traction conditions required to trigger the malfunction. No error codes were found during an informal scan, and no warning lamps or messages appeared prior to or during the failures. Component for Inspection: The vehicle is available for inspection. I believe the failure is rooted in the traction control or engine management software, which appears to improperly shut down the powertrain when it detects specific wheel-spin patterns in reverse, subsequently disabling critical safety systems including brakes and transmission locks.

  • 2025 MAZDA CX-90
    - Alpharetta, GA

    On 3/19/2026, the vehicle ignition was turned on. Immediately, we received a variety of warnings, including: "charging system malfunction", "i-ACTIVESENSE Partially Disabled", "Hybrid System Malfunction", "i-stop Malfunction", "Charging System Malfunction", and "Critical Systems Failure". Vehicle has only 20,040 miles. On 3/20/2026 8:30 AM, we contacted the Mazda dealer where vehicle was purchased new and they said "we are not sure when we can get to it--it could be several weeks". We had the vehicle towed to this dealer since they were involved in addressing the recall about "fuel sensors". An email was also sent to Mazda corporate so they would be aware of the response we received from the dealer and help with a car rental. It was very fortunate that we were not on the highway when this happened as it would have been an extreme safety risk.

  • 2017 MAZDA MX-5

    I am filing this complaint regarding a manual transmission failure on my 2017 Mazda MX-5 Miata (VIN: [XXX] ) at 55,000 miles. The failure occurred without warning while driving, causing immediate and significant transmission malfunction. Mazda issued Technical Service Bulletin 05 [XXX] c addressing this exact defect — defective synchros in 2nd and 3rd gear causing grinding, inability to engage gears, and transmission failure. My VIN falls within the TSB coverage range. The cutoff is sequential number XXX and my sequential number is XXX, placing me within the affected range. Despite this documented defect and my vehicle being under Mazda's Powertrain Limited Warranty at time of failure (under 60,000 miles), Mazda refused full coverage. The dealer offered only 50% goodwill. I escalated to the Mazda regional representative, who also declined full coverage and redirected me back to the dealer — evasive given the documented nature of this defect. This is not isolated. Multiple 2016–2017 MX-5 owners have reported identical failures, and a Mazda engineer publicly acknowledged on video that weak transmission tolerances cause gear teeth to shear under load. Active petitions are calling for a formal recall. While I was able to drive to the dealership, this failure occurred without warning. A similar failure at highway speeds or in a less favorable situation could result in complete loss of vehicle control, posing significant danger to the driver and surrounding traffic. I request that NHTSA: (1) investigate this defect across all 2016–2017 MX-5 Miata manual transmission vehicles, (2) determine whether a formal recall is warranted, and (3) require Mazda to honor full warranty coverage for VINs within the TSB range.   • 2024 MAZDA CX-90

    I am reporting a serious safety defect involving unintended acceleration in my 2024 Mazda plug-in hybrid vehicle. On multiple occasions, the vehicle has suddenly accelerated without input from me, creating dangerous situations where I nearly lost control and risked a collision. This issue has occurred at least four times since January 2026. I previously brought the vehicle to an authorized Mazda dealership, where it remained for approximately two weeks; however, they were unable to replicate the problem and returned the vehicle, stating no faults were found. Despite this, the unintended acceleration continues to occur intermittently. I have attempted to follow up with both the dealership and Mazda corporate without receiving a response. This ongoing issue poses a significant safety risk to myself, my passengers, and others on the road.

  • 2024 MAZDA CX-90

    The car had a recall repair to the steering system done due to it being sticky. But the issue has coma back again while driving on ramps and curvy roads. The steering requires significantly more effort and also has a feel of a ratchet. It also feels like a ratchet, when moving very slow and making tight turns. The car also had a recall battery replacement done for the MHEV battery and now the problem is back again within a few months and it shows "Hybrid system failed" and "Engine malfunction".

  • 2008 MAZDA TRIBUTE

    The contact owns a 2008 Mazda Tribute. The contact stated that while the vehicle was parked, the steering wheel became difficult to turn and inoperable, with the maintenance warning light illuminated. The contact stated that the steering wheel became difficult to turn. The dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 274,300.

  • 2024 MAZDA CX-90
    - Spring Lake, MI

    The 2024 Mazda CX-90 has a steering issue. The steering wheel sticks and is jumpy when turning. I took it to the dealer and demonstrated the issue by driving slowly, turning the wheel, releasing it and watching the car continue to turn while the wheel remained in the position I had left it.This anomaly was easily overcome by applying reverse steering pressure or by accelerating the vehicle. After inspection by the Mazda tech at Crown Mazda in Holland, Mi. the work summary stated in part:"This is a design matter, and not an actual problem. No problem found".
