2021 MAZDA MAZDA3- San Clemente, CA
At 31,000 miles the control arm lower ball joints are torn on both lower. They must be replaced. This is a safety hazard. This should not be an issue with low mileage on this car.

2024 MAZDA CX-90- Visalia , CA
Upon starting the vehicle, on the dash there was a message "hybrid system malfunction" as well as "engine malfunction have the vehicle inspected". Upon driving it home, the same message appeared 10/24/25 as of today when the car starts now the only message that remains is "engine malfunction have the vehicle inspected" I have an appointment at Mazda on 10/30/25 for the vehicle to be inspected. This vehicle has already been taken to this location for two recalls. Today 10/26/25 I looked up my vin for any new recalls and there is an additional recall. Manufacturer Recall Number7825I NHTSA Recall Number25V-568 Summary When fuel containing ethanol is used, it may react with materials on the sending unit circuit board in the sub-tank. This reaction can create a buildup that interferes with the gauge function that may cause the display on the instrument cluster to read an incorrect fuel level. Safety Risk If the vehicle incorrectly indicates an inaccurate fuel level, there is a risk of running out of fuel while driving. This would result in an engine stall and the vehicle cannot be restarted, increasing the risk of a crash.

2016 MAZDA CX-9- La Habra, CA
Cylinder head crack. Combustion gasses leaking into coolant causing overheat. Combustion gasses bleeding into oil causing excessive crankcase pressure.

2021 MAZDA CX-5- Marathon, WI
The contact owns a 2021 Mazda CX-5. The contact stated that while the vehicle was parked, the rear back glass fractured without impact. The contact stated that the vehicle had just been parked for several minutes prior to the failure when the contact returned from a doctor's appointment. The vehicle was not diagnosed or repaired by an independent mechanic or the dealer. The contact stated that the failure had occurred four times since June 2023. In addition, the contact stated that the rear seat was covered with shattered glass. The contact was instructed by the insurance company to contact the NHTSA Hotline to report the failure. The manufacturer was not made aware of the failure. The failure mileage was unknown.

2016 MAZDA MAZDA6- Sun Prairie, WI
The contact owns a 2016 Mazda Mazda6. The contact stated that after her husband had driven for approximately 2 hours with the seat heater activated, the passenger’s side seat overheated, and the seat started smoking. The driver stated that the seat heating element overheated and burned a hole in the seat, damaging the air bag located near the seat heater sensor. The dealer was contacted, and an appointment was scheduled. The vehicle was not diagnosed or repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 113,500.

2025 MAZDA CX-70- Vancouver, WA
Safety Issue: Gear box unexpectedly disengaged while driving at highway speeds, between 60 and 70 mph. This occurred without warning and under normal road and operating conditions. Driver was unable to accelerate or decelerate vehicle. Driver did not have forward propulsion of vehicle at time of incidents (this happened more than once), and therefore, this is an extreme safety issue.

2024 MAZDA CX-90- Monte Sereno, CA
The chrome/metal lining/coating on the Parking Break Switch peels off leaving a very sharp metal edge that cuts the finger (with bleeding) when owner tries to enable the parking break.

2018 MAZDA MAZDA3- Santa Clar, CA
What component or system failed or malfunctioned, and is it available for inspection upon request? The high-pressure fuel pump failed. This component failure prevents the car from starting or staying running. The check engine code confirmed the high-pressure fuel pump failure. The failed component is currently at the Mazda dealership and is available for inspection upon request. How was your safety or the safety of others put at risk The car suddenly stalled while driving on the roadway, creating a dangerous situation. If this had happened on the freeway or in fast-moving traffic, it could have led to a serious accident. Losing power and control of the vehicle at higher speeds poses a major safety risk to both the driver and surrounding vehicles. I was fortunate that the failure occurred on a city street rather than on the freeway. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The Mazda dealership inspected the vehicle and confirmed that the high-pressure fuel pump had failed, which was also verified by the check engine code. This failure caused the stalling and starting issues. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle was inspected by an authorized Mazda dealership, which diagnosed and confirmed the high-pressure fuel pump failure. It has not been inspected by police or insurance representatives. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? Before the final failure, the car was hesitant to start, and after it stalled, the check engine light appeared. There were no prior warning messages or signs before the car suddenly stalled while driving. Additional details: The car is only seven years old with approximately 52,000 miles, and it is concerning that such a critical component would fail this early.
