  • 2021 FORD BRONCO SPORT
    - Moore, OK

I had recalls for body control module and power train control module calibration updates. i took it to my ford dealership on august 12th, 2026 they had it done with within the day i picked it up. within a week after (i don’t drive my car much just to work mostly) i had an error come up on my dash saying the powertrain control had failed and i was unable to put my car in park. i immediately called the dealership they asked me to bring it back in. they ended up giving me a loaner vehicle and was with out my car for 2 weeks before I called and asked for an update. they said they were so sorry the transmission was compromised and i was going to have to pay $11,500 for a new one. I had never had issues with my transmission before bringing it in for my recalls to be addressed. i am still without my car and figuring out what to do next.

  • 2024 LINCOLN NAVIGATOR
    - BROOKLYN, NY

On August 31, 2026, at approximately 11:07 a.m., the vehicle had been parked and was stationary with the engine running. After remaining stationary for a short period of time, the vehicle suddenly and unexpectedly moved backward rapidly on its own. I did not intentionally command the vehicle to move backward and I did not press the accelerator. While the vehicle was moving backward, I pressed the brake pedal twice in an attempt to stop the vehicle. The brake pedal felt soft, as though there was little or no braking resistance, and the vehicle did not respond to my braking attempts. Immediately afterward, the vehicle unexpectedly moved forward and struck a vehicle in front of it. That vehicle was then pushed into another vehicle. The cause of this incident is currently unknown. I am concerned about a possible braking-system failure, unintended vehicle movement, powertrain/transmission control malfunction, electrical/electronic control malfunction, or other vehicle-control defect. The vehicle is available for inspection. I have requested that the dealer preserve all electronic and diagnostic data before any repair, reset, software update, or reprogramming is performed.

Full electrical power system failure. Heard a loud pop noise emanating from under the rear seats of the car. The power. Failure lights came on and the car lost power. We were able to get the car to slowly drive back to the garage where it completely shut down and was inoperable from that point on. Car has been towed to the local dealer for diagnostics and repair.

We were pulling camper to Nashville TN. We pulled off I24 toward I65 North. I noticed the engine was raving and it felt like I was missing gears on the transmission. I was roughly 4 miles from my destination when I noticed smoke coming from under the hood on passenger side and flames coming from under hood on passenger side of the hood. I stopped the truck/ camper. Three of us had extinguishers trying to put fire out. The entire engingine compartment burned and fire intruded into driver compartment.

  • 2024 CADILLAC ESCALADE ESV
    - Tracy , CA

On August 17, 2026, at approximately 96,685 miles, my 2024 Cadillac Escalade ESV equipped with the 6.2L L87 V8 engine lost motive power while I was driving on Highway 101 in California. The vehicle stalled, I pulled over, and I was unable to restart it. The vehicle was towed to the dealer. The dealer diagnosed a seized engine that would not rotate, with metal contamination in the oil system and internal engine damage caused by a failed bearing. During teardown, the dealer specifically documented that the rod bearing failed and caused the engine seizure. This vehicle had previously received the GM L87 engine recall remedy on August 11, 2025, at approximately 47,987 miles. The dealer documented that the required PicoScope test passed and that the vehicle was operating normally at that time. The engine failure occurred approximately 48,698 miles after the recall remedy. GM subsequently replaced the engine under warranty. I am reporting this as a safety-related loss-of-motive-power event because the engine failed while the vehicle was traveling on a major highway and could not be restarted. I request that NHTSA consider this incident in connection with Recall 25V-274 and Recall Query RQ26001 and evaluate whether the recall remedy adequately identifies L87 engines that remain at risk of subsequent failure.

  • 2023 CHEVROLET SILVERADO 1500
    - CHARLESTON, SC

The engine in my 2023 Silverado ZR2 (6.2L L87 V8) failed. It is available for inspection at Rick Hendrick Chevrolet ([XXX]). On XXX at about 67,000 miles, and the days leading up to it, the truck repeatedly struggled to start due to metal on metal contact, almost leaving me and my family stranded multiple times. Driving the vehicle produced a loud rattling noise at 2200 to 2700 rpm, and felt unsafe to drive. Especially with my family. The problem was reproduced and confirmed to be an engine failure by the Rick Hendrick Chevy dealership a week after being dropped off with an appointment. I have been told the engine needs to be replaced under the special coverage program (N252494003). The L87 recall inspection and oil change was completed on XXX also by Rick Hendrick Chevy. There were not warning lights or other symptoms leading up to this engine failure. The symptoms of struggling to start and the ticking noise as 2200 to 2700 RPM started that weekend before I brought to dealership. They got progressively worse. Dealership has said I will receive a new engine under warranty but no timeline for completion. Additionally I have not been furnished with a service loaner or option for one. Leaving me with no vehicle to replace it. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

  • 2016 FORD EXPLORER
    - Sonora, CA

Approximately 3 years ago, while driving my 2016 Ford Explorer on a [XXX], warning chimes sounded and a power steering/steering assist fault message appeared. Electric power-steering assist then stopped functioning and the steering became extremely difficult to operate. I slowed down and was able to maneuver safely off the freeway. I shut the vehicle off, opened the driver’s door, waited approximately 45 seconds and restarted it. Normal steering assist returned. Shortly afterward, I took the vehicle to Oakdale Ford in Oakdale, CA and reported the problem. I was advised that further diagnosis/investigation would involve approximately $5,600 in costs. I declined due to the expense, and no repair was performed. The cause of the failure remains UNKNOWN. The vehicle remains available for inspection. During the approximately 3 years since, the vehicle has unexpectedly lost electric power-steering assist at least a dozen additional times, at different speeds and on different types of roads. Sometimes warning chimes/messages occur without loss of assist; other times assist is actually lost. Shutting the vehicle down, opening the driver’s door, waiting approximately 45 seconds and restarting has restored steering assist each time, but the problem later recurs unpredictably. I live in a mountainous area and regularly drive narrow, winding roads with steep drop-offs and limited or no guardrails. Sudden loss of steering assist while negotiating a curve makes the vehicle significantly harder to control and creates a serious risk of leaving the roadway. No crash or injury has occurred to date. The failure is intermittent, unpredictable, repeatedly occurs while driving, and has not been repaired. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
