2025 CHEVROLET SILVERADO 1500- Woodland Hills, CA
On Thursday, July 9, 2026, at approximately 2:30 p.m., I was driving my 2025 Chevrolet Silverado 1500 Duramax on Highway 40 while towing a properly connected trailer. The Multi-Flex tailgate had been fully closed and latched before I began my trip and had not been opened at any point prior to the incident. While traveling on the highway, a California Highway Patrol officer pulled alongside my vehicle and alerted me that my tailgate was down and bouncing on the trailer I was towing. I immediately pulled over to the shoulder and observed that the entire Multi-Flex tailgate had unexpectedly unlatched and was resting on the trailer. I closed and latched the tailgate, which appeared to operate normally, and continued to my destination. At no time before or during the incident did the vehicle display a "Tailgate Open" warning or any other warning message on the instrument cluster. The unexpected opening of the tailgate caused it to strike the trailer, resulting in damage to the Multi-Flex tailgate, the rear bumper, and the trailer. The vehicle has approximately 11,000 miles, was purchased new, and has not been modified. I have opened a case with General Motors, and the vehicle is scheduled to be inspected by an authorized Chevrolet dealership.

2025 CHEVROLET TAHOE- Homestead, FL
This 2025 Chevrolet Tahoe has experienced three separate and recurring engine-related failures. The first failure occurred at approximately 41,350 miles and required replacement or repair of the lifters and camshaft. The vehicle remained at the dealership for approximately 15 days. The second failure occurred at approximately 54,619 miles and involved another lifter failure along with traction control problems. The third failure occurred at approximately 65,140 miles. The Check Engine warning light illuminated while the vehicle was being driven, and the vehicle again experienced engine-related problems. The vehicle has now been at the dealership for more than one week, and no repair or permanent solution has been provided. I am concerned that these repeated engine failures and traction control problems create a serious safety risk, particularly because the vehicle is driven on public roads and highways. Despite previous repairs, the engine problems continue to recur. I am requesting that NHTSA investigate these repeated safety-related failures.

2022 CHEVROLET SILVERADO 1500- Summerville , SC
The internal transmission Output Shaft Speed Sensor (OSS) and its associated internal circuit/lead frame assembly failed. The vehicle is fully intact, operational, and available for inspection upon request. When the Output Shaft Speed Sensor intermittently loses its signal (dropping to 0 RPM while the vehicle is stopped at red lights or in traffic), it forces the vehicle into an immediate electronic "limp mode." Upon pressing the gas pedal to accelerate, the transmission violently slams into gear with massive mechanical force. This sudden, unpredictable jerking and hesitation while accelerating into active traffic creates an immediate risk of being rear-ended, losing traction, or losing control of the heavy vehicle in an intersection. The problem was officially confirmed via a diagnostic computer scan by an authorized GM dealership (Crews Chevrolet), which pulled and documented history codes P0722 (Output Shaft Speed Sensor Circuit No Signal) and P0700 (Transmission Control System Malfunction Warning). However, the dealership declined physical teardown or sensor replacement because the electronic failure is intermittent and could not be replicated during a brief technician test drive. Yes. The vehicle's external wiring harness was physically inspected by an authorized General Motors dealership technician, who confirmed the external wiring passed inspection. This officially isolates the intermittent electrical drop to an internal transmission component failure. It has not been inspected by police or insurance. Yes. The symptoms appeared suddenly without prior warning. The transmission began shifting erratically and violently, immediately followed by the illumination of the dashboard Check Engine Light (Malfunction Indicator Lamp). The onboard computer triggered diagnostic trouble codes P0722 and P0700.

2019 CHEVROLET SUBURBAN- Angier, NC
The contact owns a 2019 Chevrolet Suburban. The contact stated that the middle row seat belts failed to retract, and the seat belts became inoperable. The contact had taken the vehicle to an independent mechanic and to two dealers, and the contact was informed that the seat belts could not be replaced because parts were no longer manufactured. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 120,000.

2017 CHEVROLET SILVERADO 1500- Vernon, CT
Torque converter clutch failed and sent metal chips through the transmission causing the transmission to fall and made the truck not drivable. The check engine light came on a few hours before the transmission finally gave out. The transmission would not shift out of first gear. We were on a family trip to [XXX] from [XXX] and this left us stranded with three kids in the truck on the side of the road. When I took the truck to the shop they asked several other transmission shops if they had seen this issue and they all confirmed they did with the same truck and the best case is to rebuild the transmission instead of replacing the just the torque converter clutch due to the amount of damage it did. 2022 CHEVROLET EQUINOX- Herkimer, NY
The contact owns a 2022 Chevrolet Equinox. The contact stated that while driving at an undisclosed speed and accelerating, there was an abnormally loud rattling sound coming from the muffler. The vehicle was taken to the local mechanic, and it was determined that the baffles inside the muffler were loose, and the muffler needed to be replaced. The vehicle was not yet repaired. The dealer was notified of the failure, but no assistance was provided. The manufacturer was not contacted. The failure mileage was 11,000.

2019 CHEVROLET EQUINOX- McDonald, OH
The contact owns a 2019 Chevrolet Equinox. The contact stated that while driving approximately 65 MPH, the brake assist warning light illuminated. The contact stated that the brake pedal was depressed but was stiff. The brake pedal was pressed with force to slow the vehicle. The contact pulled over to the side of the road, and the message "Engine Power Reduced" was displayed, and the check engine warning light illuminated. The vehicle was towed to a dealer, where it was diagnosed and determined that the mechanical vacuum pump had exploded and there were metal shards inside the engine oil, camshaft, and the timing jump, and that the engine was blown and needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was unknown.

2021 CHEVROLET SILVERADO 1500- Elbing, KS
The contact owns a 2021 Chevrolet Silverado 1500. The contact stated that while driving at an undisclosed speed, the message that the rear seats were occupied and that the windows were rolled down was displayed, which required the contact to press the OK button. The vehicle was taken to the dealer where it was purchased, but no assistance was provided. The traction control and the ABS warning lights were illuminated while depressing the brake pedal. Additionally, the cruise control mode was inoperable. The contact replaced the brake pedal position sensor independently, and the vehicle was taken to an independent mechanic, who reprogrammed the sensor. The contact stated that after the repair, the brake lights erroneously illuminated while the vehicle was turned off, causing the battery to drain. The contact disconnected and reconnected the battery, which reset the vehicle; however, the failure recurred. The vehicle was not further diagnosed or repaired. The manufacturer was not contacted. The failure mileage was 110,881.
