  • 2023 CHEVROLET BOLT EUV
    - Asheville, NC

I was taking a left turn from [XXX] onto [XXX] in the outer (right) lane. As I was exiting the turn, the AEB went off suddenly without warning -- the car came to a sudden and complete stop. There were no cars or obstacles in the road ahead of us in either lane and so it is unclear what would cause the braking event. Due to the sudden stop, a man driving a moped was not able to stop in time--he was able to swerve to avoid a head on collision, but he collided with our rear passenger bumper. While, fortunately, there were no injuries, both vehicles sustained minor damage. This is the second incident of "phantom braking" I have experienced with this car. The first incident was within the first year of owning it driving on a 35 MPH road when it mistakenly thought a car was veering into our lane (oncoming traffic) -- in this case the phantom braking was brief and did not bring the vehicle to a stop. My 2022 Chevrolet Blazer abruptly Shut down all power for about five seconds while I was stopped at a red light. The vehicle restarted after I pressed the push-button ignition, but the dashboard displayed a brake system error warning with no explanation of the cause or instructions for next steps. No other warning lights preceded the shutdown. The vehicle appeared to resume normal operation after restart, but the brake system warning remained. This appears consistent with complaints filed by other 2022 Blazer owners describing simultaneous electrical and brake system failures.

Failure - B000A-00 Body Control Module Error code - Shift Lever Position not plausible . Safety - This causes the car to upon attempting to turn off the car say "Shift into Park" on the dashboard and the car refuses to turn fully off. This can cause the battery to drain excessively from repeated constant start/stop attempts and leaves the car in an impossible state. You cannot safely leave the car in this state but you have no choice until you get lucky enough to hope an attempt is recognized as parked and shut off normally. This could jeopardize safety in situations you need to turn your car off Official Bulletin Number: GM TSB 23-NA-119 There is no warning prior to failure, and no way to override it

Remedy for campaign 14V047000 was executed defectively, creating an unsafe mechanical failure that destroyed the powertrain. Failure occurred while or shortly before vehicle stalled in heavy traffic. Vehicle would not restart, but ignition switch stuck in start position while attempting restart, causing starter motor to crank continuously until the switch was turned to the off position. It is believed that starter motor was running while engine was running since last successful start about 3 minutes prior to stall, interfering with normal operation of the engine and eventually causing engine to jump timing and sustain catastrophic internal damage. Subsequent GM dealer service visit found that a burr on the new key provided with campaign 14V047000 repair caused ignition switch to bind in start position. Service staff buffed out burr on key which resolved switch binding, but vehicle refused to start. Borehole inspection of engine found damaged valves and pistons, leading to diagnosis of jumped timing. No other work other that diagnosis has been attempted at this point. Estimate to replace engine is $8600

  • 2024 CHEVROLET SILVERADO 3500
    - Strasburg, CO

The infotainment/control module in my 2024 Silverado 3500HD failed unexpectedly while I was traveling cross-country. The module is available for inspection upon request. Prior to the failure, I did not receive any warning lamps or messages. When the system failed, I lost access to multiple camera systems integrated into the vehicle, including backup and visibility-assistance cameras used to aid lane changes, parking, maneuvering, and identifying obstacles around the truck. Due to the size and height of this vehicle, these cameras provide important visibility of areas that are difficult to observe directly, including pedestrians, cyclists, and children who may be below the driver's line of sight. The loss of these camera systems reduced situational awareness and increased the risk of a collision while backing, changing lanes, or operating the vehicle in crowded areas. The failure also disabled navigation, hands-free functions, audio, WiFi connectivity, and access to vehicle settings controlled through the infotainment system. The vehicle was inspected by a Chevrolet dealer in Cape Cod, Massachusetts. The dealer attempted a software reflash to correct the issue; however, the procedure rendered the unit completely inoperable. The dealer confirmed that the module required replacement and quoted an estimated repair cost of approximately $1,500. The required replacement part was unavailable, and despite contacting numerous Chevrolet dealerships, I was unable to obtain a repair during my trip. As a result, I was required to continue operating the vehicle while towing a large trailer for an extended period without the camera systems and related functionality. I have also become aware of GM Service Bulletin PIC6630B, which indicates that GM has identified this failure mode and that it is not an isolated occurrence. Given that a single module failure can disable multiple safety features, I request that it be evaluated by an independent authority accordingly.

The contact stated that while his son was stopped at a gas station, while buckling the seat belt, the floor anchor pretensioner cable snapped off. The contact stated that there was no warning light illuminated. A Chevrolet dealer was contacted and stated that the VIN was previously under recall, and the vehicle was repaired in Maryland in 2016 under NHTSA Campaign Number: 16V209000 (Seat Belts). The vehicle was recently repaired at Lancaster Chevrolet in Lancaster, SC (1616 Charlotte Hwy, Lancaster, SC 29720), and the contact stated that the vehicle would be covered under warranty if the mileage was under 184,700 miles. The contact paid out of pocket to have the cable replaced. The cause of the failure was not diagnosed. The manufacturer was not notified of the failure. The approximate failure mileage was 200,000.

I needed a replacement driver-side windshield wiper arm for my 2023 Chevrolet Silverado 1500 LTZ Z71 with factory rain-sensing wipers. GM's electronic parts catalog directed the Chevrolet dealer to part #23387855. The dealer physically compared the part to my original arm and confirmed it was the wrong shape and would not fit. The dealer opened a formal Catalog Omission/Error (COE) ticket with GM. I was then informed that GM redesigned the wiper arm on my truck. The correct redesigned arm has been ordered, but I was told there is no estimated availability date and that it is not currently available through GM dealerships or GM's parts distribution/warehouse system in North America. This is a safety concern because a damaged or failed wiper arm could leave an owner without proper windshield visibility during rain or severe weather. I am also concerned that the passenger-side arm may have the same catalog and availability problem, although this has not yet been confirmed. I am requesting that NHTSA investigate whether this issue affects both wiper arms and other 2023 and newer Silverado 1500 trucks with this wiper configuration nationwide.
