Subject: Vehicle Safety Incident Report – Fuel Pump Disconnection and Oil Leak To: National Highway Traffic Safety Administration Date: July 27, 2026 I am submitting this report to detail a recent vehicle malfunction that posed significant risks to my safety and that of others on the road. While driving to work, I experienced a situation where my vehicle was unable to shift gears properly or accelerate as expected. This loss of acceleration and responsiveness created substantial control problems, making it difficult to maneuver safely in traffic. Notably, no dashboard warning lights were illuminated at the time, which gave me the false impression that the vehicle was operating correctly. Initially, I perceived a dragging sensation in the car, which I believed was manageable. However, the situation quickly escalated when I observed smoke coming from the vehicle. The presence of smoke not only impaired my visibility but also posed a serious risk of fire or engine failure. The danger was heightened as I attempted to drive uphill; the car struggled even more, increasing the potential for an accident or collision due to impaired maneuverability. Following this incident, I had my vehicle towed to an auto repair shop for a diagnostic check. The diagnostic report revealed that the fuel pump was not attached to anything and that oil was leaking into the pan. These findings confirm that the issues were real and not just perceived. The disconnected fuel pump and oil leak represent systemic failures, which could have had catastrophic consequences if not addressed promptly. In conclusion, this experience highlighted grave risks to both personal and public safety. I urge vehicle manufacturers and regulatory bodies to prioritize detecting and preventing such malfunctions.

Hello [XXX], I was wondering are you handling the case against Hyundai/Genesis. I own a 2022 GV70 43K on it, took it to the dealer on 7/24/26 photos on file at the dealer in Dublin, Ca. Got home in the evening, and parked my car in my garage, then left the next evening 7/25/26 to a wedding got home late, 7/26/26 drove my card to get a bite to eat and seen this, (see photos) I reviewed on line and this a known issue with Hyundai/Genesis a well documented factory defect. That the paint just lifts off due to no being cured properly. That it prematurely oxidizes and weakens and peels off, mine is not oxidized but it just has peeled straight off the hood. This is like toyota white tacoma Trucks issues, with paint. Are you handling this and where do I get in on the claim or start one? for a 60K vehicle this is unacceptable. I also have a video. My# is [XXX] if you need to reach out. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

This supplements my prior complaint, ODI #11751961, for the same vehicle. Following that complaint, an authorized dealer performed front camera and front radar ADAS calibration on 7/21/2026 and returned the vehicle as operating as designed; the manufacturer's representative confirmed in writing on 7/22/2026 that the camera had been within specification. On 7/23/2026, approximately 1:21–1:35 PM, driving in the left lane of a divided parkway with a concrete median barrier, with HDA2 and Lane Following Assist both active and indicating green, the steering wheel turned the vehicle toward the concrete barrier at least seven times, requiring manual correction each time. This occurred both with a lead vehicle approximately 4 seconds ahead and with no vehicle within more than 10 seconds ahead. Earlier the same day, driving the opposite direction with both systems active and green, the vehicle applied hard automatic braking twice with no vehicle within more than 10 seconds ahead (traffic present only in the adjacent lane); I had to manually accelerate because a following vehicle was close behind. My original complaint reported the automatic braking failing to activate; the system now also activates falsely. Additionally, the hands-on-wheel warning repeatedly illuminated while my hands were on the wheel, taking approximately 30 seconds to detect them. I have stopped driving the vehicle. The manufacturer was notified in writing the same day, and a second repair visit is being arranged. No crash or injury has occurred.

The vehicle has experienced a pattern of recurring electrical failures beginning shortly after purchase, involving driver door controls (window, lock, mirror, and interior lighting functions), which have required four separate repair attempts over approximately three years, including replacement of a door module, a wiring harness, and a door switch assembly. Despite these repairs, the same driver door electrical failure has recurred multiple times. The vehicle has also experienced a seat memory module failure (module was replaced), and a persistent rear hatch malfunction that has never been resolved. The most serious concern is that the vehicle has, on multiple occasions, unexpectedly shut down while being driven, including instrument cluster and infotainment screens going dark, occurring when the automatic engine stop/start feature engages at intersections. The vehicle has restarted on its own after acceleration in some instances. Power steering assist has also been affected during at least one of these events. Most recently, the vehicle failed to start at all on one occasion, requiring roadside assistance to jump the battery; the dealer diagnosed unstable battery voltage but did not identify a root cause connecting this event to the prior shutdown incidents. This creates a significant safety risk due to potential loss of vehicle control or loss of power while operating in traffic. The problem has been reproduced and confirmed by the dealer on multiple occasions (door module and wiring harness failures were diagnosed and repaired), though the shutdown-while-driving events specifically have not been duplicated by the dealer despite multi-day testing. The vehicle has been inspected exclusively by the authorized dealer; it has not been inspected by police or independent parties. Warning symptoms (flickering door lights, dead door controls) preceded the shutdown/stall issue by roughly a year before the stalling symptom began.

When Highway Driving Assist / Lane Following Assist is active — confirmed by illuminated green HDA and lane assist indicators — the vehicle applies uncommanded steering torque, to the left or to the right, strong enough to force the vehicle out of its lane. This occurred on every drive, daily, from delivery on 7/2/2026 (approximately 23,200 miles) until the vehicle was brought in for service on 7/13/2026. The behavior was identical regardless of lane markings (solid, dotted, or absent) and road geometry (straight or curved), at highway speed and on local roads at approximately 30 mph. On multiple occasions the vehicle was pushed toward highway barriers (concrete median and metal guardrail, on both left and right sides) and into the oncoming traffic lane; avoiding barrier contact or returning from the oncoming lane required forceful manual correction, during which the rear of the vehicle oscillated side to side before directional control was fully regained. This occurred regardless of speed. Separately, the forward collision-avoidance / automatic emergency braking system failed to activate on multiple occasions when approaching stopped or slowing traffic, requiring hard manual braking by the driver. No warning lights displayed. An authorized dealer inspected the vehicle 7/13–7/21/2026: scans found no stored diagnostic trouble codes, road tests did not reproduce the behavior, and the dealer performed front camera and front radar ADAS calibration on 7/21/2026. I am currently monitoring the vehicle to determine whether the calibration corrected the behavior. A comparable loaner vehicle of the same brand exhibited correct HDA/LFA operation, which is how I confirmed my vehicle's behavior is abnormal. No crash or injury has occurred.

On XXX, I delivered my 2020 Genesis G70 3.3T AWD Prestige (64,267 miles) to Jenkins Genesis of [XXX], for routine factory-recommended maintenance, including transmission, transfer case, front differential, rear limited-slip differential (LSD), brake fluid, and cooling system services. Before this visit, the vehicle had no history of drivetrain problems. It had recently completed an approximately 8½-hour drive from [XXX] to [XXX], followed by three weeks of normal driving, without any binding, vibration, shuddering, abnormal turning characteristics, warning lights, or drivability concerns. The dealership retained the vehicle for four days for what was represented as a one-day service. On XXX, I was told the vehicle had been road tested and was safe for release. **Immediately after taking delivery**, the vehicle developed severe drivetrain binding, violent shuddering, heavy vibration, and significant resistance during normal turns, creating an immediate safety hazard. I returned directly to the dealership. After initially stating they could not duplicate the condition, the dealership ultimately diagnosed **both rear CV half shafts as failed** and prepared an approximately $4,500 repair estimate. The dealership later performed a second rear LSD fluid service **without my knowledge or authorization** after concerns were raised about the original service. They initially offered to replace both rear CV half shafts at no cost, but later withdrew that offer and returned the vehicle to me with both rear CV half shafts still broken. I respectfully request that NHTSA investigate whether improper drivetrain service procedures, incorrect lubricant application, inadequate post-service inspection, or another safety-related condition can cause catastrophic driveline failure immediately following routine maintenance. I am also concerned that the forces sufficient to fracture both rear CV half shafts may have damaged other AWD drivetrain INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I am writing to file a formal complaint regarding the service I received, and the dealership’s failure to address what I believe is a serious safety issue following recall repairs performed on my vehicle. On XXX, I brought my vehicle for a routine oil change and to complete three mandatory recall campaigns, including an ECM software update related to a misfire recall. Prior to this service, my vehicle had never exhibited any misfiring, transmission, or drivability issues. However, on Sunday, XXX, and again on Monday, XXX my vehicle began experiencing dangerous driving symptoms that started only after the recall work was completed. Specifically: The engine RPMs surge while accelerating, but the vehicle does not increase speed accordingly. The vehicle jerks when braking. The vehicle hesitates and feels unsafe while driving at highway speeds. These issues did not exist before Genesis performed the recall repairs. On XXX, I contacted my service advisor, to report these safety concerns. He assured me that a manager would contact me ASAP and determine the next steps. No one ever called me back. I called again that same day. During that conversation, Rick suggested that my worn brake pads and rotors were the reason my vehicle was “misfiring.” This explanation makes does not explain why my engine RPMs are flaring and the vehicle is failing to accelerate properly immediately after an ECM recall update. Once again, I was told someone would follow up, and again, no one did. On XXX, at approximately 9:00 AM, I called Genesis of Danvers again and spoke with Rick who told that if I dropped the vehicle off today, nothing could be done with it today, and that a manager would call me back. Got vehicle towed XXX. As of today XXX I still have no update on my vehicle. No phone card no email no loaner. I am becoming increasingly concerned by the lack of communication surrounding a vehicle that was towed because of a significant safety concern. I had the known reported issue TSB 25-BE-004G repaired and even after repair there is a burning wire smell that the dealer claims they can’t replicate. We had the same issue with Genesis Plano initially and after driving the vehicle out of state we were stranded and took it to Genesis of Peoria where the issue was diagnosed. We are encountering the issue again and it is a fire risk hazard.
