    I was driving to Castle Rock, CO from Parkville, MO on [XXX] for vacation. I used the heated seat feature off and on during the trip. When I arrived in CO at my destination I noticed my thighs were hurting- stinging and hot. I checked my thighs and saw they were red and blisters were forming. I went to urgent care and I referred to the ER- I was given information about how to treat 2nd/3rd degree burns.   • 2016 MAZDA CX-5
    - Reading, MA

    Mazda issued a safety recall (Mazda safety recall 4320A, NHTSA campaign number 20V-063) to address daytime running light flickering/failure. At the time, only the passenger side daytime running light was replaced under the recall by Mazda, with a gasket/gas absorption sheet being installed on the driver's side. The driver's side daytime running light is now flickering/dim. Mazda did not repair the safety defect properly on the driver's side for the daytime running lights under Mazda safety recall 4320A, NHTSA campaign number 20V-063, because Mazda only installed the gasket/gas absorption sheets that have since failed, causing the daytime running lights to flicker, increasing the risk of crash. Mazda has refused to cover the cost for replacement of the daytime running lights per safety recall notice 4320A / NHTSA 20V-063, even though replacement of these lights is covered by the recall. Both a Mazda dealer and Mazda corporate customer service have been contacted regarding this issue and both have refused to provide the remedy as outlined in the safety recall. Mazda has provided an estimate of $1,466 to replace the daytime running lights.   • 2019 MAZDA CX-9

    I am reporting a serious safety issue involving a defective suspension part (lower control arm) manufactured by Dorman and purchased from an online auto parts retailer. On October 7, 2024, I ordered a Dorman lower control arm for my vehicle and had it professionally installed on November 7, 2024 at Johnston and Magwood Wheels & Tires. On November 8, 2024, at approximately 5:40 p.m., while I was driving and made a hard brake, I suddenly lost steering control and my front left wheel collapsed. Fortunately, I was the only one in the car and managed to avoid an accident. My vehicle had to be towed to a nearby shop (Kal Tire) and was later sent back to the original shop for further inspection. On November 12, technicians confirmed that the newly installed lower control arm had failed, which caused serious damage to the suspension. I paid $1,774.10 in repairs in order to make the car safe to drive again. This is extremely dangerous — if I had been driving on a highway or with passengers, it could have resulted in a major accident or injuries. When I contacted the retailer about the warranty, they only offered to refund the part itself and refused to take responsibility for the damages caused by this defective product. I believe this part should be investigated for safety defects. When I searched online, I noticed Dorman has had similar recalls on control arms, which makes me even more concerned. I am reporting this incident to NHTSA in hopes that you will look into this part to protect public safety.

  • 2011 MAZDA MAZDA6
    - Crystal Lake, IL

    The contact owns a 2011 Mazda Mazda6. The contact stated that while her mother was driving at slow speeds in a parking area, there was an abnormally loud metallic sound coming from underneath the vehicle, and moments later, the vehicle became extremely difficult to maneuver. No warning lights were illuminated. The vehicle was towed to the dealer, who determined that the rear crossmember had detached from the vehicle due to excessive corrosion of the subframe. The contact was informed that the vehicle could not be repaired. After investigating the failure, the contact related the failure to NHTSA Campaign Number: 19V323000 (Structure, Suspension); the VIN was previously included, but was repaired in 2019. The contact stated that the recall repair was for the front passenger’s side lower control arm. The manufacturer was notified of the failure, a case was opened, and the contact was referred to the NHTSA Hotline for assistance. The failure mileage was 150,000.

  • 2021 MAZDA CX-5
    - Saint Petersburg, FL

    The contact owns a 2021 Mazda CX-5. The contact stated that while driving approximately 50 MPH in an intense rainstorm, the windshield became foggy. The contact activated the HVAC system to clear the windshield, but only hot air was coming from the vents. The contact stated that the hot air made the foggy windshield worse. The contact used a towel to wipe the inside of the windshield. The contact had taken the vehicle to a local dealer; however, the failure was not duplicated. The vehicle was not repaired. The contact stated that the failure occurred during intense rainfall. The manufacturer was informed of the failure and referred the contact to the NHTSA Hotline to report the failure. The failure mileage was approximately 35,000.

  • 2024 MAZDA CX-5

    The contact owns a 2024 Mazda CX-5. The contact stated that while his wife was entering the garage, at an undisclosed speed, the vehicle suddenly accelerated; the brake pedal was applied but failed to respond, and it crashed into the refrigerator. The contact was concerned that the failure could have occurred while children were at play. No warning lights illuminated. There were no reported injuries, airbag deployments, fires, or police reports filed. There was a tire imprint on the garage floor. The contact called the local dealer, who scheduled an appointment. The vehicle was not diagnosed or repaired. The manufacturer was contacted, who opened a case and confirmed there was no recall coverage. The failure mileage was approximately 10,000.

  • 2024 MAZDA CX-90

    While I understand that tires themselves are typically covered by the tire manufacturer’s warranty, this situation is different. Mazda equipped the CX-90 with Falken OEM tires that are inadequate for the weight and design of the vehicle, leading to premature wear at only 20,000 miles. This is not normal wear-and-tear. My claim is that Mazda bears responsibility for installing unsuitable tires on a heavy SUV, which created a safety hazard.

  • 2022 MAZDA CX-5

    Several times and most recently today 8/18/2025 at 5:30 PM my 2022 Mazda CX-5 accelerated while I had my foot on the brake and only the brake. Hard pressure on the brake kept the SUV from lurching forward. This has happened before but I did not think it would occur again. But it has and I find that this could easily cause an accident. The SUV in question has only 23,000 miles on it. An investigation and recall to fix the problem should be initiated.

  • 2025 MAZDA MAZDA3

    January 10, 2025, at low city speed, I struck a small pothole. The entire car felt like it had been in an accident—a violent jolt—followed by immediate vibration and a pull. Inspection showed one OEM 18" rim was bent. I contacted Mazda after diagnosing and was offered a slight discount on the replacement rim, not a warranty repair or full diagnosis. I have the bent rim in my possession. July 2025, at low speed, I barely contacted another small pothole and immediately felt the same drivability issues. A second rim was found damaged. This was only 4,500 miles after the first incident. I called the dealership manager; he said he would get back to me but never did. I later sent a letter that went unanswered. My subsequent research shows innumerable similar owner complaints for 2020–2025 Mazda 3s experiencing rim deformation from ordinary road irregularities. Why this is a safety issue (technical points): Bead-seat deformation & air loss: A bent alloy rim can distort the bead seat, causing slow leaks or sudden deflation, especially after heat cycles—creating a loss-of-control risk. Dynamic instability: A rim that’s even slightly out-of-round produces high-frequency vibration, steering pull, and reduced traction in turns or under braking; it can trigger ABS/ESC interventions and increase stopping distance. Load path & suspension stress: The 18" low-profile package leaves little sidewall cushion. On normal bumps/potholes, impact energy transfers directly to the wheel. Repeated “bottom-out” events may damage shocks/struts, alignment, and tires, compounding instability. Repeatability & foreseeability: Two events under ordinary use within a short interval indicate a systemic integrity problem, not random abuse. Requested resolution: A no-cost remedy: (1) replace all rims/tires with more durable parts and inspect/repair suspension; or (2) provide comprehensive wheel/tire protection and repair damaged rim; or (3) exchange he vehicle for a comparable model with no issue
