I am writing to report a serious safety concern related to a recent over-the-air software update for my 2025 Volvo XC90. On March 10, 2026, I was prompted to install a software update and scheduled it to complete overnight. The following morning, the vehicle interface indicated that the update had been successfully completed. Additionally, during a routine service visit on March 12, 2026, dealership personnel also believed the update had been properly installed. However, on March 15, 2026, while driving, I experienced a critical and dangerous failure: my brakes became unresponsive, and I was unable to stop the vehicle using the brake pedal. Multiple warning lights appeared simultaneously. Fortunately, I was near my home and was able to safely bring the vehicle to a stop by coasting through nearby streets. This situation could have resulted in a catastrophic accident had it occurred on a highway or in heavy traffic, particularly as my daughter was in the vehicle at the time. The vehicle was subsequently towed to the dealership. I was informed that during the initial installation attempt, the battery voltage dropped below the required level, causing the update to pause. Critically, I received no notification that the update had failed or was incomplete, and the vehicle interface continued to indicate that the update had been successfully installed. Upon inspection on March 16, 2026, the dealership determined that the software update resumed and completed installation on March 15—while I was actively operating the vehicle. At that time, the vehicle lost communication with its electronic systems, resulting in the loss of braking functionality. This sequence of events raises a significant safety concern. A software update that can pause due to low battery voltage and later resume without any notification—particularly while the vehicle is in operation—creates an unacceptable risk to drivers, passengers, and the public.

I was driving home in my VolvoXC60. I think its a 2025 or 2024 model. I drive very safe and never do anything crazy when I drive. I came to a stop like normal at a red light and the screen said accident detected (specifically rear collision avoidance or rear collision something) and the seatbelt started crushing my chest and my body. It just continued to press down and crush me and the car wouldn’t let the seatbelt unbuckle like it was locked up. It was locked and basically just sucking her in with a very strong force. No car got close to me and I finally got it after like 2 minutes and drove home. I think its pretty unbelievable a new suv which is supposed to be very safe and reliable does that and appreantly and my chest is still very sore. Both dealerships near us don’t have a loaner car for a month. It was very scary and basically the car was crushing me for multiple minutes.

Despite multiple repair attempts by Volvo Cars Sugar Land — including software reloads, module reprogramming, and the completion of Recall R10333 — the vehicle continues to experience failures that directly compromise my safety and the safety of my wife. I no longer feel safe operating this vehicle. My XC40 has undergone five documented repair visits, including: • Electrical system failures • Infotainment instability • Parking sensor and camera malfunctions • Multiple software reloads and module reprogramming • Recall R10333 “Total Upgrade” On March 11th, while driving at approximately 60 mph on the highway, the IHU screen froze, went completely black, and all audible safety indicators inside the cabin were lost, including: • Turn signal click • Hazard flasher sound • Parking sensor tones • Multimedia audio This condition lasted for nearly 10 minutes, during which I had no ability to safely signal lane changes or rely on audible warnings. I was forced to continue driving until I could safely pull over. After parking, shutting the vehicle off, and waiting several minutes, the IHU restored itself. This incident occurred five days after the most recent repair attempt and represents a serious safety hazard and a complete loss of trust in the vehicle’s reliability.

Three (Right Front & Rear, Left Rear) of four factory-installed Pirelli Scorpion Zero All Season tires (255/40R21, DOT 1XB RM582P 3923) are exhibiting tread block chunking, cracking, and physical material separation. All four tires share an identical DOT code from a single production run. Failure modes include lateral cracks across multiple tread blocks, physical loss of tread block material in multiple locations, and one tire with a deep tread void consistent with delamination. At 36,176 miles on a 2024 vehicle, this level of structural failure represents a safety hazard. The tread separation creates risk of sudden air loss or blowout at highway speed. I am aware of other NHTSA complaints filed for this exact tire size and model describing identical failure modes. I am requesting NHTSA investigate whether a safety defect exists in this production batch.

My tire somehow got a nail in it. I drove straight to Volvo when noticing the issue around 730am on 3/13/26. At 745am I spoke with [XXX] at Schumacher Volvo, he advised that they were too busy today to assist me. I advised I was afraid to drive with this and needed help and he told me to buy a tire lock kit and I could do it at another tire store. When I went to the parts department they advised the brake lock would not work and the only way to fix my tire would be to have Volvo remove the special lock nuts or replace them with common ones. Of course x they had no time to do this so they left me with zero options or ability to fix my car. I was forced to drive 30+ minutes home with zero solution. Completely unsafe and unfair I have absolutely no way to resolve this issue with my car. Unacceptable and scary considering Volvo claims they are all about safety but no concerns letting a single mother and her 2 children drive in a tire with a nail. The driver's side mirror repositions itself downward, upward, or outward when unlocking and opening a door. It infrequently repositions while driving as well. These occurrences put me and others at risk by increasing my blind spot when I need to change lanes. Two Volvo dealers have reproduced the mirror changing positions when unlocking and opening the door, but a few times they were not able to get the car to reproduce it. Technicians have performed software updates, deleted and reprogrammed the keys to the car, and performed a factory reset on the infotainment center to no avail.
