  • 2021 CHEVROLET COLORADO
    - San Diego, CA

    I am reporting a serious safety defect involving my 2021 Chevrolet vehicle. While driving, multiple warnings appeared including “Service Power Steering,” “Steering Assist is Reduced,” “Service ESC,” and “Service Battery Charging System.” The vehicle then lost steering assist, braking performance became unreliable, and it created a serious risk of crash. The vehicle later shut down completely and has remained inoperable. The failure appears electrical and affected multiple critical systems at once, suggesting a cascading system malfunction. Since the incident, the vehicle has not restarted or operated normally. I have replaced the battery, alternator, starter, ignition switch, brake vacuum pump, ECM, spark plugs, fuses, and relays, but the issue persists. The vehicle still shows charging system faults and severe battery drain, indicating an unresolved underlying electrical defect. I have found numerous reports from other 2021 Chevrolet owners describing nearly identical failures, including sudden loss of power steering assist, ESC warnings, charging system failures, battery drain, and complete vehicle shutdowns. This suggests a possible widespread defect affecting this model year. Given the loss of steering assist, braking irregularities, and full vehicle shutdown while driving, I request NHTSA investigate this issue for a potential safety-related defect and determine whether a recall or manufacturer action is necessary.

  • 2023 CHEVROLET SILVERADO 1500
    - Swarthmore, PA

    Transmission went out while driving on highway, entering limp mode. Ended up needing a valve body replacement at only 20k miles which was covered, but now I am having more transmission issues still with delayed forward engagement and slipping gears and it is only at 65k miles. Seems to be a lot of customers having the same issue. Took months for the truck to be fixed as "many other trucks" were waiting on the same part according to dealership.

  • 2017 CHEVROLET SUBURBAN

    What component or system failed or malfunctioned, and is it available for inspection upon request? The fuel delivery system has failed. Specifically, the fuel lines are leaking at the connection point on the top of the fuel tank. The vehicle and the affected components are currently available for inspection upon request. How was your safety or the safety of others put at risk? This defect poses an immediate fire and life-safety hazard. Because the fuel leaks while the vehicle is powered on, any spark or heat source could ignite the fuel tank, leading to a vehicle fire or explosion. Additionally, the cabin environment has become toxic; the concentration of gasoline vapors is so high that it causes physical illness. One of my children vomited due to the fumes. The vehicle is currently undrivable because the vapors are intoxicating and hazardous to the health of all occupants. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, the leak has been inspected, reproduced, and confirmed by an independent service center. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No, the vehicle has not yet been inspected by the manufacturer, law enforcement, or insurance representatives. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? There were no warning lamps or dashboard messages (such as a Check Engine Light) prior to or during the failure. The symptoms—specifically a strong odor of raw gasoline—first appeared four days ago. The intensity of the odor and the rate of the leak have increased significantly since the initial onset.

    The bottom of driver seat leans to the left and moves when stopping or accelerating, moving forward or backwards, and does not stay stabilized like it should. This issue is not an electrical or power issue but seems to be a broken support bolt that attaches to track for seat.

  • 2021 CHEVROLET EQUINOX

    On way to [XXX] car started hesitating stop at Oreily’s they hooked up said 3rd cylinder was misfiring took to Bruner Chevrolet I told them what the read out said on the 3rd cylinder misfiring they said catalytic converter was messed up and changed it out drove car home still hesitating went back to Bruner Chevrolet I asked them about the recall on the fuel injector module they said my car was not on the recall said it needed the spark plugs and fuel injectors maintenance they did all that then they test drove the car engine light came on said the fuel injector needed to be replaced cost me 866.33 for all this what I want to know if the fuel injector module wasn’t working properly would it cause all of this my car only has 40,800 miles on it INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

  • 2019 CHEVROLET SILVERADO 3500
    - Redding, CA

    The contact owns a 2019 Chevrolet Silverado. The contact stated that on several occasions at various speeds, the contact observed an exhaust odor coming into the cabin of the vehicle from the vents. The vehicle was taken to two dealers where the failure was not able to be replicated. The manufacturer was notified of the failure. The failure mileage was approximately 60,000.

  • 2021 CHEVROLET SILVERADO 1500
    - Winter Garden , FL

    2021 Chevrolet Silverado 1500 Custom 2.7 L Turbo 4 cylinder with 34,000 miles When Truck cranks and put in gear begins moving at low speed between 1-2 gear (low speed 5-10 mph forward and also in put into reverse) hard jolted severely injuring passenger. Felt like being rear-ended by another car. Passenger was hurt & required Trauma 1 EMS & 2 surgeries (Spinal Fusion Surgery) hospitalization from 12/2/2025 to 12/13/2025. The truck jolt directly caused this accident and there were many recalls for this truck yet we had no direct knowledge of this information & was never notified of any of these issues. Truck was services at same dealership just weeks prior to accident. The truck check engine light came on only after the accident. It happened on a private gated property. Only select trucks have been said to do this very hard jolted injuring passenger and owners are not made aware using Silverado 1500 trucks. Please acknowledge receipt of this report and advise.   • 2018 CHEVROLET SILVERADO 1500
    - Klamath Falls, OR

    A known radiator issue, with microcracks in radiator is producing a leak and will require replacement. There are many examples of this online for the model years 2014-2018, and are known by the manufacturer. I am having mine replaced with 95,000 miles on my truck.

  • 2023 CHEVROLET BLAZER

    The contact owns a 2023 Chevrolet Blazer. The contact stated that while driving, a code appeared on the dashboard P0128. When the vehicle's engine was switched off, the engine cooling fan continued to run. Warning light stating the steering would be disable.; however power steering was not lost. The vehicle was taken to a dealer where it was diagnosed with a thermostat failure. The manufacturer was notified of the failure. The approximate failure mileage was 48,000
