2023 CHEVROLET SILVERADO 1500- Salem, OR
The contact owns a 2023 Chevrolet Silverado 1500. The contact stated that while stopped at a traffic light, the engine turned off and the instrument panel became inoperable. The vehicle was restarted; however, the failure persisted. The vehicle was taken to the dealer; however, no cause for the failure was found. The contact stated that on a separate occasion, the message "Tailgate Open" was displayed on the instrument panel. The contact pulled over to the side of the road and, upon inspection, noticed that the tailgate was closed. The vehicle was taken to the dealer; however, no cause for the failure was found. An employee at the dealer closed and opened the tailgate several times until the message disappeared. The contact stated that on a separate occasion, the radio stopped functioning, the driver's door unlocked unintendedly, and the driver’s side seat belt failed to retract as intended. The vehicle was taken back to the dealer, where it was diagnosed that the seat belts had failed. The contact seat belts were replaced. Most recently, there was an abnormal sound coming from the engine. In addition, the brake pedal was depressed, and the vehicle failed to respond. After a while, the vehicle was able to stop. The message "BRAKE System Failure” and “Replace Brake Pads and Rotors" was displayed. The vehicle was not diagnosed or repaired. The contact stated that the failure persisted. The manufacturer was made aware of the failure and referred the contact to the NHSTA Hotline for assistance. The failure mileage was approximately 22,000.

2023 CHEVROLET BOLT EV- Los Angeles, CA
The Chevy Bolt steering wheel has a skip track and previous track button mounted to the left-rear side of the steering wheel. These buttons often become unresponsive for extended periods of time (up to 30 minutes) when a media device is connected. When pressed, they will display an error on the instrument panel screen that says “Action Unavailable While Indexing”. Although media controls are not explicitly safety related, the frustration caused by these unresponsive buttons can cause the driver to become distracted or look for unsafe methods of adjusting the media playback that the buttons are supposed to perform.

2015 CHEVROLET EQUINOX- Dayton, OH
While driving my 2015 Chevrolet Equinox (2.4L Ecotec) in active traffic, the vehicle suffered a sudden, catastrophic loss of oil pressure with no prior warning, displaying a 'Stop Vehicle' message. The sudden loss of engine power created an immediate collision risk. Independent diagnosis confirmed the engine lost all oil. This is a known defect (GM Special Coverage #14882) where a frozen/clogged PCV orifice causes excessive crankcase pressure, blowing out seals (like the rear main seal and VVT solenoids) and dumping the oil. I called GM, who told me I needed the vehicle to be towed to a local dealership (White Allen Chevrolet) refused to perform the GM-mandated Manometer Vacuum Test to confirm the PCV defect, attempting to conceal the root cause of the engine failure. This is the only test I asked them to perform, and I provide my GM case number and the TPS number. This known PCV design flaw creates a sudden, highly dangerous stalling condition at highway speeds. It had been very cold in Dayton for over a week, something known to cause this problem.

2025 CHEVROLET BLAZER- Hamburg, NY
On two separate occasions the vehicle lost all power while driving. On December 5, 2025 turning on to a main road from parking lot the vehicle shut off with no prior warning lights. Shifted vehicle to park. No power to anything, unable to unlock doors, unable to turn on hazard flashers, stuck in the middle of the road at night. Waited a minute and pressed the start button and vehicle started with no problem. Again, on February 24 , 2026 making a left turn in intersection, without any warning the vehicle shut off… no power at all. Stuck in middle of road with no lights , no hazard flashers ,unable to get out of the car. Few minutes later vehicle started again. The vehicle did not stall while stopped, it completely lost power while moving. Reported problem to West Herr Chevrolet of Hamburg NY , brought vehicle in to dealership on February 25, 2026 and they were “unable to duplicate the concerns” The dealership paperwork states that the vehicle stalled while stopped, this vehicle lost all power while it was moving. We now have the vehicle back in our possession but are extremely concerned for our safety and the safety of others.

2020 CHEVROLET SILVERADO 1500- Ajo, AZ
The contact owns a 2020 Chevrolet Silverado 1500. The contact stated that while driving approximately 5 MPH and exiting a parking lot, the rear wheels locked up as if the brake pedal was depressed. There was no warning light illuminated. The vehicle was stopped and shifted to park(P), and the vehicle was turned off and restarted several times to reset the vehicle; however, the rear wheels were still locked up. The contact disconnected the battery cables and was able to unlock the rear wheels. The contact stated that the failure was intermittent, but was a recurring failure. The contact had taken the vehicle to a local dealer, where it was not diagnosed, and the vehicle was not repaired. The contact researched online and related the failure to NHTSA Campaign Number: 26V083000 (Power Train); however, the VIN was not included in the recall. The manufacturer was not informed of the failure. The failure mileage was approximately 92,000.

2019 CHEVROLET EQUINOX- Conway, SC
The instrument cluster and driver information center is completely blank, and none of the steering wheel controls are operating. Vehicle starts and runs normally, but no indication of speed, turn signals, headlights, gas level, etc.

2023 CHEVROLET BLAZER- Sylvania, OH
The radiator developed a leak at around 36k miles and needed to be replaced. My understanding is that this is an ongoing problem with this vehicle.

2022 CHEVROLET SILVERADO 1500- Crawfordsville, IN
The contact owns a 2022 Chevrolet Silverado LTD 1500. The contact stated that the tailgate was opening unintendedly while driving and while the vehicle was parked. There was no warning light illuminated. The dealer was notified of the failure, but no assistance was provided. The vehicle was not diagnosed or repaired. The contact related the failure to an unknown recall. The manufacturer was not notified of the failure. The failure mileage was approximately 130,000.

2021 CHEVROLET SILVERADO 2500- Marion, IN
While traveling at highway speed on the interstate in my 2021 Chevrolet Silverado 2500 HD (approx. 120,000 miles), the vehicle experienced a sudden hard jolt followed by a momentary rear wheel lockup. This occurred without warning. Immediately after, the transmission defaulted into a fail-safe condition and became locked in 4th gear. The vehicle would move but could not shift properly, resulting in abnormally high RPM and engine load at highway speeds. I was forced to continue driving several miles in this condition to reach a safe exit. The unexpected drivetrain behavior and temporary wheel lockup created a serious safety risk, including potential loss of control and risk of collision. The dealership identified transmission diagnostic trouble codes P2715 and P2733 (pressure control solenoid performance). Transmission fluid was found to be burnt and contaminated with metal debris. The vehicle is subject to GM Special Coverage N242454441 for a known control valve body defect that can cause harsh shifting and reduced engine power. Although code P0747 (referenced in the bulletin) was not present, the failure involved transmission hydraulic pressure control. The dealership is replacing the valve body under the special coverage but is not addressing the internal transmission damage, stating they cannot determine whether the damage resulted from the covered defect. The sudden rear wheel lockup and transmission fail-safe condition at highway speed present a significant safety concern involving unintended drivetrain behavior.

2020 CHEVROLET SILVERADO 1500- Lake Charles, LA
Internal oil consumption. With no oil puddled or spattered under the vehicle at time of purchase from a dealership or since then, a dealer certified vehicle with AFM at 153 miles from leaving the dealer the oil was non-existant on the dipstick. This directly indicates a TSB, Techical Service Bulletin involving the cause of failed lifters causing internal oil consumption and damage to lifters, cam shaft, push rods, rings and possibly more. This is a common result of the Active Fuel Management system which shuts off some of the cylinders for fuel economy for Chevrolet V6 and V8 engines. The problem was reported to the dealer by me imediately after the oil check at 153 miles and then towed to the dealer. As this is considered a catastrophic amount of oil loss, the vehicle could've been rendered inoperatable during operation in traffic and been a servere safty hazard. The dealer after initial inspection sent me a message that they think the problem is an undetectable small leak so they put some dye in the oil to check it. Which evades the actual internal consumption problem of 1.5 to 2 or more quarts of oil lost in just 153 miles by that ratio that would amount to 13 quarts per 1,000 miles. And helps them stall for time, to try to run out the very short 3 month/3000 mile drivetrain warranty. There was noticable erratic engine performance and some shudder but that shudder may have come from the transmission. The odometer read was 115690 at purchase and 115843 when towed=153 miles driven from when the certified inspection was performed for a loss of 1.5 to 2 quarts that I know of, it could have been much more oil missing. A suddenly inoperatable vehicle in town at intersections or on the highway at high speeds could be deadly for myself or others in traffic. I took and have videos of checking the dipstick to show it dry but it's an mp4 file which I don't see a place to upload it here. Available upon request.
